In accordance with department procedures, performs activities pertinent to issuing, receiving and general maintenance of medical records or X-ray films. Keeps related records.
May perform any combination of the following:
Answers telephone requests for information, records, films; follows up with hospital personnel to retrieve same or to request information required for patient files. Operates Kardveyer and Lektriever to locate patient identification cards and corresponding unit numbers; issues numbers as requested. Issues and records new numbers as required; changes and/or corrects information on cards; purges card files periodically. Types data into data base by way of computer terminal or other direct entry device; maintains related files. Receives and assembles lab and X-ray reports, graphs, records from other hospitals and the like, following established sequence for inserting into patient's file. Reviews patient file to ensure required documents and signatures are included. Ensures receipt of files of discharges and deceased patients; maintains follow up until records are completed and received. Maintains related logs and statistical records.
High school plus 1 year of related work experience; or equivalent combination of education and experience.
