Description

Will perform case management to coordinate and optimize the health care interventions for the injured worker across the continuum of care. To assess, implement, coordinate, monitor, and evaluate the options and services required by the injured worker to meet health and human service needs. To advocate, communicate, and manage resources that promote quality and cost-effective interventions and outcomes for the injured worker for the Office of Risk Management.  Will travel to physician's offices in Middle Tennessee.

Key Functions and Expected Performances:

 

Coordinates/Supports the care of the injured worker

         Coordinates the immediate and ongoing care of the injured worker when referred by the Office of Risk Management

         Provides needs assessment and assist in developing appropriate treatment plan

         Systematically monitor injured workers' progress and ensure that they are following the recommended medical treatment plan

         Assesses whether alternative medical care services and requests for medical equipment are indicated and make recommendations to the treating physician/treatment team

         Assesses the cost effectiveness of treatment and/or durable medical equipment

         Formulates a plan for return to work with the treating physician, the injured worker, and the Occupational Health Clinic's Return to Work program coordinator

         Attends physician appointments with the injured worker to develop a working relationship that facilitates optimal outcomes and maximal medical recovery

         Facilitates and promote open communication among the medical treatment team, injured worker, adjuster, and Return to Work program coordinator.

         Educates and direct the injured worker to appropriate departments to resolve issues impeding their recovery and/or return to work

         Serves as resident expert for faculty and staff regarding case management issues

Coordinates workers' compensation processes

         Assists in the development of processes and systems to properly reflect injured workers' current status

         Utilizes Vanderbilt software systems for information and communication (EPIC, StarPanel, VOHIS, STARS, etc.)

         Develops and maintain Corporate Health Services case management policies and procedures

         Acts as consultant to the Office of Risk Management to provide updated physician information for state required panels

         Utilizes Department of Labor online reporting system as required

         Provides ongoing assessment of case management files to determine need for Medical Director intervention

Provides education and support

         Provides education and support regarding VUMC return to work program to treating physicians and ancillary service providers

         Provides education to the injured worker regarding medical treatment options and clarify treatment plan

         Provides education to the injured worker regarding the VUMC return to work program, EAP options, and other services available

         Assists in dissemination of Department of Labor changes and updates

         Promotes Vanderbilt physicians and ancillary services

         Assists in orientation of new staff

Professional Development

         Participates in conferences/in-services for continued education requirements for Certified Case Manager and networking opportunities (MidSouth Workers' Compensation Association, Middle TN Case Managers Assoc., etc.)

         Develops proficiency in software programs utilized in case management

         Obtains/evaluates/synthesizes new information and knowledge/research applicable to workers' compensation

 



Basic Qualifications


Preferred Education, Skills, and Experiences:

         Graduate of an accredited discipline specific program and 3 years of experience or the equivalent required

         Current RN licensure required

         Certified Case Manager Certification required

         Experienced in Worker's Compensation required

         Onsite case management experience preferred

         Ability to travel throughout Middle Tennessee

         Med/Surg or Orthopedic RN experience a plus

         Strong organizational and time management skills

Registered Nurse
