Job Summary

Represent a cooperative process between the patient and case management service providers.  Increase patient adherence to diagnosis and treatment recommendations.  Monitors and supervises patient progress throughout program and during follow-up.  Works independently to accomplish the following:  1) interacts with patients and healthcare providers to ensure ongoing care of patients with abnormal cancer screenings; 2) works with patients to identify and overcome barriers to screening and follow-up care; and 3) responsible for data management related to patients’ cancer screening, diagnosis and treatment.

Requirements

Bachelor’s degree plus 2-3 years related program experience or equivalent combination of education and experience.  Experience in clinical or human service setting with familiarity with medical terminology.  Familiarity with Medicaid managed care a plus.  Excellent written and oral communication skills; excellent customer service, organizational, time management, and interpersonal skills; ability to work independently and as part of a team; ability to work effectively with people of different cultural and economic backgrounds; proficient in MS Office software.  Bi-lingual preferred, but not required.  Some weekends and evenings required.  Must possess a valid NYS driver’s license and have reliable transportation.
