Ambulatory Care and Academia Fellowship with Midwestern University College of Pharmacy, Glendale Campus

Job Description: This is a 1-year fellowship in ambulatory care and academia with Midwestern University College of Pharmacy, Glendale Campus (MWU-CPG) in collaboration with Banner Ambulatory Care Services and Cardiac Solutions. The purpose of the fellowship program is to develop competence and expertise so that the individual can function independently as an ambulatory care clinical pharmacist and academician. The individual will work under a collaborative practice agreement to assess patients, manage medication therapy, order labs, and provide pertinent patient education related to covered disease states including diabetes, weight management, hypertension, lipid management, asthma, COPD, tobacco cessation, heart failure, and hypotension. The academic experience will provide opportunities to refine presentation skills, small group facilitation, item writing and precepting skills. MWU-CPG provides the primary site where the fellow completes academic responsibilities; whereas Banner Health Ambulatory Care Services and Cardiac Solutions are where the fellow will hone his/her skill in clinical practice. The fellow will divide their time up as follows: 40-50% in clinical service, 30% in teaching, 10% in service to the college and University and 10-20% in research, based on fellows’ interest.

The fellow will develop 1) Clinical practice skills through longitudinal rotations at Banner Ambulatory Care Services and Cardiac Solutions; 2) Teaching skills with students (through the advanced pharmacy practice experiences (APPE), didactic lecturing, and workshops) and clinicians (through in-service lectures to healthcare professionals); 3) Research skills essential for productivity whereby the fellow is responsible for developing 1 research proposals for which he/she will act as the primary investigator and complete manuscript submissions as a primary author. 4) Service skills through involvement within CPG, the Arizona Pharmacy Association and American Society of Consultant Pharmacists.

Upon completion of the fellowship program, the fellow will be a competitive candidate for clinical or academic positions with expertise in ambulatory care.

Credentials and experience of applicants:  Required: PharmD. Completion of a PGY-1 ASHP accredited (or pending) program or ASHP approved equivalent experience preferred. Commitment to excellence in teaching and patient care, and eligibility for pharmacy licensure in Arizona are required. The qualified candidate must be strongly committed to interdisciplinary education and teamwork.
