Position Description
Employees in this classification are experienced LVN's responsible for performing a wide variety of patient care activities in an assigned specialty or subspecialty outpatient clinic. Work is performed under the direction of a staff nurse and/or physician as part of a total health care team in providing complete evaluations to patients. Works under the direct supervision of staff nurse or physician. No delegated supervisory authority; may instruct co-workers and supervise them in the performance of special procedures. Work is performed under usual clinic conditions, necessitating a high degree of contact with patients, families and clinic personnel.

Major/Essential Functions

The duties of a Senior Licensed Vocational Nurse (SR LVN) include completing annual nursing core competency training, maintaining inventory of supplies, and ensuring proper equipment maintenance. Additionally, the SR LVN ensures the clinic complies with all policies, procedures, and programs such as ACO/TIPPS. Works closely with the Medical Director and Head nurse to supervise and train staff in their assigned clinic. Orders supplies. The SR LVN identifies patient needs for referral to resources and facilitates continuity of care. They assist with or perform exams and procedures, documenting interventions in the Electronic Medical Record (EMR), and accurately taking and recording vital signs and measurements. This role involves assessing the physical and mental health status, needs, and preferences of culturally diverse patients and their families. The SR LVN communicates patient data via EMR to support decision-making, identifies alterations in patient responses, and collaborates with the healthcare team to develop patient-centered care plans. The SR LVN implements teaching plans for patients and families with clear health learning needs communicates with patients per clinic protocols, prepares necessary equipment and instruments for exams and procedures, and rooms patients.

Required Qualifications
Education:  Completion of a Board of Nursing accredited program of vocational nursing.
License/Certification:  Current Vocational Nursing license in the state of Texas or the attainment of a temporary permit from the Board of Nursing to practice vocational nursing pending full licensure in the state of Texas.
Experience:  At least two years of LVN experience in a hospital, clinic, or community-based environment.
