Job Description:

The Student Health Care Center (SHCC) at the University of Florida in the Department of Community Health and Family Medicine (CHFM) seeks a full-time Clinical Programs Coordinator II. Join our team at the SHCC, where you'll play a pivotal role in maintaining and enhancing our case management (CM) system to ensure seamless care for our patients. As a Clinical Programs Coordinator II, you'll collaborate closely with all SHCC providers to facilitate continuity of care, follow-up, and referral services. Your responsibilities will include documenting CM services in medical records, assisting UF students with social service needs, coordinating clinical information sharing among care providers, and liaising with external psychiatric care providers and programs. Additionally, you'll be instrumental in conducting assessments, evaluating services, and ensuring patient satisfaction.

This position requires excellence in delivering clinical case management services and emergency intervention as necessary. The Clinical Programs Coordinator II collaborates with SHCC providers to maintain and monitor a case management system to provide continuity of care, follow-up, and referral services for patients, including those at high risk. The individual will substantiate case management services rendered by documenting them in the medical record. As time permits, provide outreach, program development, oversight, and teaching/training of faculty members, staff members, and other employees. Develop, maintain, and provide professional collaborative consultative services with Human Resource Services (HRS), faculty members, staff members, other employees, residents, fellows, supervisors, colleges, departments, and SHCC clinicians. Consult with the administration to enhance individual and organizational productivity, teamwork, and a psychologically healthy workplace. Perform other duties and functions as assigned by the Director. The individual must be detail-oriented, have the ability to adjust priorities quickly, possess innovative problem-solving skills, and have the ability to work interdependently with many levels of staff. Excellent organizational, interpersonal, and communication skills are required in order to develop and maintain liaison relationships across campus and in the community.
  • Assist students with social services needs, including referrals to community counselors, psychiatry providers, medical specialists, and university resources.
  • Provide crisis intervention and consultation with SHCC providers as needed.
  • Maintain medical records and ongoing case management with follow-up.
  • Coordinate sharing of clinical information with those involved in students' care to ensure appropriate services are received.
  • Collaborate on case management services with hospitals, mental health programs, and clinics for appropriate discharge planning and community referrals.
  • Assist with the academic withdrawal process and assess student stability for SHCC services.
  • Work closely with the SHP case manager to optimize case management services across SHCC.
  • Collect data, document in medical records, and maintain accurate departmental statistical records.
  • Submit necessary reports and surveys.
  • Write letters for patients to UF departments regarding academic functioning affected by mental health conditions.
  • Act as liaison and provide consultation with UF and community programs and organizations to optimize student care.
  • Provide short-term therapy services with comprehensive documentation.
  • Conduct ongoing assessments and evaluate services, including patient satisfaction surveys and needs assessments.
  • Schedule: Monday through Friday from 8:00 a.m. to 5:00 p.m. Hours and days will vary due to the Student Health Care Center's needs (rotating shifts could be required for Sunday clinic in fall and spring semesters). Additionally, a hybrid option of working from home two days per week after six months of employment is possible.

Expected Salary:

$46,850 - $58,000 Annually

Minimum Requirements:

Bachelor's degree in an appropriate area and two years of relevant experience or an equivalent combination of education and experience.

Preferred Qualifications:
  • Degree in social work, counselor education, counseling/clinical psychology, or related discipline with two years of relevant experience
  • Licensure in the State of Florida with an appropriate governing body (e.g., Florida Board of Psychology; Board of Clinical Social Work, Marriage and Family Therapy, and Mental Health Counseling)
  • Experience in college mental health, including crisis assessment, brief intervention, confidentiality requirements, and referrals
  • Knowledge of local, state, national, and international social service resources
  • Familiarity with crisis intervention, referral techniques, and accessing community resources for successful referrals
  • Understanding of psychiatric conditions contributing to clinical presentations
  • Knowledge of unique social service needs of college/university populations
  • Proficiency in case management strategies to optimize social support for patients
  • Expertise in clinical case management
  • Administrative skills
  • Computer application skills for effective implementation and maintenance of a case management and tracking system
