Department Information
Stamps Health Services is an outpatient ambulatory care center that provides health care and health education to students and their spouses/domestic partners. The center is located in a state-of-the-art, 40,000-square-foot facility within the Joseph Brown Whitehead Building, next to our Campus Recreation Center. Stamps Health Services provides care in six areas: Primary care, Women¿s health, Psychiatry, Travel/Immunization/Allergy, Pharmacy, and Diagnostic Services (lab and radiology). Primary Care Clinic is the largest of the clinics at Stamps Health Services and sees approximately 24,000 visits/year.

Job Summary

Provide a range of patient-centered services that links patients with health care, psycho-social and other services to ensure timely, coordinated access to medically appropriate levels of health and support services, continuity of care, ongoing assessment of patient's and/or family members' needs and personal support systems. This position will interact on a regular basis with: Staff, students, family members, and off campus providers.This position typically will advise and counsel: Patients.This position will supervise: N/A

Responsibilities

Job Duty 1 -
Provide comprehensive assessment of the patient's treatment needs and personal support systems.

Job Duty 2 -
Maintain ongoing dialogue with supervisor and other health providers to ensure effective implementation of health treatment plan.

Job Duty 3 -
Maintain and utilize a referral database, and coordinate care with other service providers involved in patient's treatment plan.

Job Duty 4 -
Periodic re-evaluation and revision of the plan as necessary over the life of the patient, which may include patient specific advocacy and/or review of utilization of services.

Job Duty 5 -
Perform other duties as assigned

Required Qualifications

Educational Requirements
Bachelor's Degree in social work, psychology, nursing, or related human services field or equivalent combination of education and experience

Other Required Qualifications
Overnight travel required for project delivery; flexibility in on-site daily service delivery hours degree in social work, psychology, nursing, or related human services (due to client shift schedules, project requirements).

Required Experience
Two to three years of job related experience

Preferred Qualifications

Additional Preferred Qualifications
Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW), Registered Nurse (RN), Licensed Mental Health Counselor (LMHC), Licensed Professional Counselor (LPC), or Certified Case Manger (CCM) Experience with facility-based and/or outpatient psychiatric disorder treatment center preferred

Preferred Educational Qualifications
Master's Degree in social work, psychology, nursing, or related human services field

Knowledge, Skills, & Abilities

SKILLS
This job requires demonstrated ability to communicate, problem solve, and work effectively with people; excellent organizational skills with the ability to manage multiple priorities simultaneously; strong analytical skills; knowledge of applicable state and federal regulations; in depth knowledge of case management tools; ability to take initiative and see tasks to completion; excellent verbal and written communication skills; skilled at establishing and maintaining positive and effective work relationships with coworkers, patients, and providers; ability to maintain confidentiality and comply with Health Insurance Portability and Accountability Act (HIPPA); knowledge of office related computer applications.

Salary range: $25.22-$27.22 / hourly
