POSITION TITLE: Case Manager—Health Navigator

DEPARTMENT: Student Success & Engagement

REPORTS TO: Executive Director of Student Care and Retention

 

POSITION SUMMARY:

The Case Manager – Health Navigator role provides case management providing support services for students in attaining their educational, interpersonal, and academic goals. The Case Manager – Health Navigator provides support to Dominican Undergraduates with specific focus on their social and academic integration into Dominican University. Utilizing the Dominican University case management platform and Unite Us platform, this position will provide wrap around holistic support plans for students having experienced an emergent barrier to their wellbeing and implement structured support plans to ensure comprehensive care has been coordinated.  This role is focused on removing barriers to care by identifying critical resources for students, helping them navigate through health care services, university systems, and promoting client health. They work closely with students’ external care team, which may include doctors, nurses, and other clinical staff to support positive client health outcomes. 

 

DUTIES AND RESPONSIBILITIES:

Intensive Case Management: 

  • Manages a dedicated caseload of Assessment and Care Team and Student Care Network student cases, ensuring coordination of care and resolution of barriers impeding student well-being.  
  • Manages a caseload of students having experienced hospitalization, medical withdrawals and oversight for their coordinated follow up care  
  • Design focused reintegration plans in coordination with the Assessment and Care Team that support academic supports as well as social emotional supports. Coordinates the with students care team on management of their discharge summary.
  • Supports the coordination of communication follow up to key continents in the students circle of support 
  • Supports administrative decision making and assessment on eligibility for student medical withdrawal  

 

Resource Navigation:

  • Manages the Unite Us platform supporting student resolution of social and structural determinant of health equity gaps
  • Manage programs that provide students technology access, including short term laptop loans, long term laptop loans, and community partner laptop purchasing programs
  • Serves as a one stop shop for student internal and external referrals
  • Provides consultation for students on topics including but not limited to Medicaid applications, SNAP, housing and food insecurity resources as well as other resources to address well-being.
  • Partner with wellness center on external mental health and case management partnerships
  • Connect students in crisis to internal and external mental health resources.
  • Collaborate with external care teams to support students' mental health outcomes.
  • Providing psycho-educational outreach programming related to mental health awareness and support.

 

Required Qualifications:

  • Masters degree in social work, counseling, student development or related field
  • Experience with case management, access and disability services, and basic needs resources assessment and navigation (i.e. local and state housing support resources, SNAP and other federal benefits, health care for uninsured)
  • Experience working with a diverse study body and designing supports to meet student needs
  • Excellent organizational skills and ability to manage time and tasks independently to meet work objectives
  • Ability to take initiative and follow through with minimal supervision
  • Ability to maintain confidentiality in all matters related to the position

 

Preferred Qualifications:

  • Experience with student support case-management technologies like Pharos360
  • Understanding of college student development theory
  • Previous experience with HSIs or minority serving institutions

 

Physical Demands:

  • Ability to spend long durations of the workday sitting at a desk and working on a computer
  • Must be able to lift up to 15 pounds at times

 

Salary and FLSA Status

  • Salary: $53,940 - $64,000 annually
  • FLSA Status: Exempt 37.5 hours
