The Community Health Nurse conducts nursing health assessments, health education, care planning and nurse care management of health related issues for clinets in the HeartWell program as well as link community health resources between patient, family members and the primary physician.

EDUCATIONAL REQUIREMENTS/PREFERENCES:

Baccalaureate degree in nursing or graduation from a NLN accredited school of nursing.

Evidence of formal post education in physical assessment.

Licensure/Certification/Registration: Valid nursing license in Maryland. Valid driver's license preferred.

EXPERIENCE/REQUIREMENTS:

• Two (2) years of cardiovascular nursing.
• Experience in community systems, entitlements, and third party reimbursement. Fluency in Spanish, Chinese, Vietnamese or Korean preferred.
• MPH or extensive experience working in a public health environment is a plus.
