Job Summary:

Position Summary:
The Health Access Network (HAN) at OSU-Center of Health Sciences, inpartnership with the Oklahoma Healthcare Authority, is working to improveaccess, quality, and continuity of care to SoonerCare members in the State ofOklahoma. The RN Case Manager offers case management/care coordination topersons with complex health care needs and contributes to the five foundationalservices in the Health Access Network: facilitating Members' access to alllevels of care utilizing case management/care coordination services,telemedicine services, electronic health records, improved access to specialtycare services and expanded quality improvement strategies.

Environmental Hazards:
Possible exposure to hazardous material, hot and cold, blood bornepathogens.

Physical Requirements:
Ability to lift and carry 8-10 pounds, stoop, reach, stand, walk, finger,grasp, feel, talk, hear, see, and perform repetitive motions with or withoutreasonable accommodations.

Education & Experience

Position Qualifications:

Required:
-Associate's degree in Nursing plus 2 years nursing experience OR,Bachelor's degree in Nursing plus 1 year of experience as an RN OR,Master's degree in Nursing OR, Master degree in Social Work plus 1 year ofexperience as an LCSW.
-Current & active RN, or LCSW licensure issued by Oklahoma State Boards ofNursing or Social Work.
-Valid Driver License.
-Case management/outpatient clinic experience.
-On Dec. 7, 2021, a federal judge temporarily blocked the implementation of thefederal contractor vaccination mandate and its associated guidance. Followingthe injunction, Oklahoma State University has, consistent with applicable law,paused the vaccination mandate.

Skills, Proficiencies, Knowledge:
Knowledge and skills in chronic disease management. Knowledge of national,state and local health and social service provider agencies; exceptionalinterpersonal communication skills - to include writing/documentation skills.Critical thinking skills and sound judgment; exhibit initiative and ability towork independently and within a team; ability to meet deadlines and prioritizeworkload; exhibit cultural sensitivity and the ability to work with diversepatient population and professional staff, faculty and medical students withinacademic health care setting. Computer skills.

Preferred:
-A minimum of 3 years clinical practice experience
-Case management certification (or eligible to obtain).
