Job Description Summary

Counseling Services within Student Health and Counseling at the University of Pennsylvania seeks a referral coordinator/case manager with experience in college counseling and/or equivalent settings. Student Health and Counseling (SHaC) is a multidisciplinary group of licensed clinical providers and support staff that care for students during their academic journey, within Wellness at the University of Pennsylvania.

The individual in this position provides referral coordination and mental health case management, matching Penn students to mental health services outside of Student Health and Counseling where appropriate. Clinical case management responsibilities may include managing care for a student until they can be referred to a more appropriate facility or provider. The individual in this position also provides consultation and collaboration with staff clinicians regarding client referral needs, insurance options, and community provider or agency availability. The Referral Coordinator/Case Manager organizes referral data, manages data collection and prepares reports. This position will coordinate and work closely with a second SHaC Referral Coordinator/Case Manager.

Counseling Services is seeking a candidate with a solid understanding of the impact of marginalization, inequality, and identity on students and a solid understanding of policies and procedures regarding principles of confidentiality.

Job Description

The Referral Coordinator/Case Manager provides support for students who need assistance finding counseling, other university resources, and access to community providers or organizations. The role requires assessment and crisis management skills to address the case management of complex mental health concerns. This position will provide follow-up case management functions through consultation with SHaC clinicians, working closely with the other Referral Coordinator/Case Manager, consulting as needed with campus partner offices, and ensuring therapeutic engagement with community providers as needed. This individual will develop and coordinate care plans for students, organize follow-up treatment plans following hospitalizations, and will meet with students periodically on an individual basis as needed until a referral out can be effected.

Principal Responsibilities and Duties:

  • Provides mental health referral and case management services, matching Penn students to clinically appropriate mental health services outside of Student Health and Counseling to other university resources and/or community providers or organizations, as appropriate.
  • Develops and coordinates care plans for students, organizes follow-up treatment plans following hospitalizations, session terminations, and requests for specialized treatment in the Philadelphia area, within the U.S. and internationally. This process may entail assessment of placement options. Collaboration with professionals within the center and in key partnership offices is essential.
  • Manages The WellTrack community provider database, including signing up and communicating with community providers, and liaising with database administrators. The individual provides consultation and collaboration with SHaC staff regarding client referral needs, insurance options and community provider and agency availability.
  • Develops, organizes, and implements referral data, including new outside providers, data collection, and reports. The Referral Coordinator/Case Manager will work closely with other the Referral Coordinator/Case Manager to accomplish this and to provide coverage for all students who need these services.
  • The individual in this position may be asked to assist in the processing of medical leaves of absence.

The role demands strong referral and case management skills as well as good organizational abilities in order to assist students presenting with high acuity or complex presenting concerns.

Position Qualifications:

  • Master’s degree with 3 to 5 years of experience in a university counseling center or mental health agency addressing the needs of a young adult population. Minimum LSW or LPC degree.
  • Clinical experience must include familiarity with referral services and case management functions and strategies including knowledge of resources in the community, stabilization options for clients, collaborative communication with multi-disciplinary providers both internally and externally, and a solid knowledge base of a wide range of mental health concerns.
  • The successful candidate will be an individual who can promote teamwork, be flexible with attention to detail, and possess the ability to communicate with a high degree of professionalism.
  • Ability to manage multiple tasks in a fast-paced work environment is a must.

Counseling Services seeks candidates with a solid understanding of how marginalization, inequality, and identity impact students. Foundation in multicultural counseling is strongly preferred. Experience with multidisciplinary collaboration, consultation, and integrated care is desirable. The individual in this position will be expected to adopt some flexible hours during the week to cover evening shifts. Some Saturdays, 10 am to 3 pm, may be required. Must be licensed in the State of Pennsylvania. This is a full-time, salaried, exempt position.
