Community Outreach Program Advisor
The Department of Surgery is seeking a motivated, empathic Community Outreach Program Advisor to join our clinical and research team focused on patient navigation for African Americans with advanced cancers. The overall role of the COPA will be to address barriers to high quality palliative care through education, support, and patient engagement. In this position, the COPA will work with patients, caregivers, and healthcare professionals. In addition to building familiarity with end-of-life concepts and issues, the COPA will receive formal training related to 1) understanding their role in reducing health care disparities; 2) working within the medical center, in collaboration with clinical support staff, to overcome challenges related to patients obtaining appointments; 3) telehealth support; and 4) communication skills specific to palliative care support.

Specific Duties & Responsibilities:
  • Activities for this position will include, but not be limited to: screening and enrollment for eligible community programs; patient mentorship; patient education; patient follow-up and reinforcement of treatment plan tasks; coordination of appointments and transportation; and patient engagement.
  • The COPA I will foster cohesive relationships with staff and clinical teams, with community partners and with colleagues associated with Dr. Fabian Johnston's research.
  • This position will work with an interdisciplinary team of physicians, nurses, care managers, social workers and other community health workers to improve health outcomes.
  • The COPA I will be active in community engagement activities to include working with community organizations and assisting with community-based activities. May direct the tasks of volunteers.
  • The COPA I must have reliable daily transportation for work related travel (to and from community site, patient homes, meetings, clinics, etc.). If using personal vehicle for work related travel, the COPA must have appropriate license and insurance


Minimum Qualifications (Mandatory):
  • Bachelor's Degree in public health, teaching and education or related field.
  • Three (3) years' related experience in patient navigation, community organization, and other similar functions.
  • Additional education can be substituted for experience or additional experience may be substituted for education requirement to the extent permitted by the JHU Equivalency.*
  • Must be able to speak, read and write in English.


*JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for the required education on the same basis. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job.

Preferred Qualifications:
  • Certification as a Navigator preferred.

  • New employees must complete all required training within 30 days of hire unless otherwise noted.
  • Requires working knowledge of social and health issues. Familiarity with medical terminology beneficial.
  • Work requires excellent interpersonal and communication skills to efficiently work with diverse groups of community members and to coordinate with team members.
  • Work requires some critical thinking skills to effectively assess social/health needs of members and to arrange for appropriate services required to meet those needs.
  • Must be able to recognize situations that require professional or clinical intervention.
  • Work requires excellent organizational skills, flexibility, and the ability to get things done.
  • Must be compassionate and sensitive to diverse situations, results-oriented, a self-starter and capable of making appropriate decisions independently.
  • Must be able to enter data on a computer.
  • Must have attitude and skills that facilitate patient engagement in their health care.
  • Must have a basic level of customer service responsiveness with telephonic experience.
  • Requires the ability to establish and maintain effective working relationships with both internal and external customers.
  • Requires analytical skills to effectively address and evaluate the health education needs of customers.
  • Demonstrates ability to work independently and efficiently.

Starting Salary Range: $61,550 - $84,700 annually (commensurate with experience)

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
