- Work collaboratively with practice stakeholders to deliver integrated mental health in a primary care setting. Provide short-term mental health treatment to targeted group of patients. This includes developing comprehensive initial assessments, treatment and discharge plans. Refer patient to higher level of care (e.g. specialty treatment) -or community long term care - as appropriate. Coordinate and revise treatment plan with input from interdisciplinary team and primary care provider. Provide routine patient evaluation/measurement that ensures that treatment progress is being made. Manage patient referrals, including coordinating care for those patients not meeting criteria for clinic mental health services. Support clinic in developing community and campus referral sources as needed. Work collaboratively with clinic staff to ensure efficient scheduling and placement of referrals. Other responsibilities, appropriate to this position, as may be deemed necessary.
Qualifications

- Master’s degree in Social WorkCurrent Colorado LicenseThree (3) years of professional clinical experienceMust possess and maintain a valid State of Colorado Driver’s License
- Three or more years of experience independently delivering mental health care in a higher education medical/clinical environment.
- Experience working as part of an interdisciplinary care team.
- Experience working with diverse populations including but not limited to complex medical conditions and social/economic issues; Home Care, Medicaid and Medicare populations.
- Experience and knowledge of HIPAA rules and regulations, including limits of confidentiality.
- Experience working with an electronic medical record system.
- Clinical knowledge and skills that demonstrate understanding of DSM V and allow for appropriate diagnosis and assessment.
- Clinical skills which are well-rounded and encompass a variety of evidenced-based modalities and theoretical orientations and include knowledge and experience with assessment, safety planning and appropriate follow-up with those patients who pose imminent risk to self and/or others.
- Some familiarity with psychiatric medications.
- Must possess excellent written and verbal communication skills and be proficient in writing of mental health evaluations, progress and discharge notes.
- Intermediate skills with word processing and developing spreadsheets using Microsoft Office such as Word, Access and Excel.
- Excellent interpersonal and organizational skills with the ability to prioritize, multitask, and follow-up.
