The role you’ll contribute:

The Director of Community Care oversees the operational development and deployment of a people centered care delivery model improving the quality of life for individuals and groups by providing wholistic, efficient and effective care across a seamless continuum. This is a divisional position (CFD-North & South) that leads the Community Care program to meet Population Health goals. The Director of Community Care has the authority and responsibility for planning, organizing, implementing, controlling and evaluating administrative functions and all operations for connecting and integrating modular components into a seamless system of care across the Central Florida Division. This work, though directed by Population Health, aligns and directly connects with the regional strategic priorities of the CFD and the Clinically Integrated Network (CIN). The Community Care program is comprised of high risk/high utilizer patients in our community with health-related social needs. The Director of Community Care must demonstrate the ability to analyze and interpret complex problems and develop effective courses of action that are consistent with the organizational mission and strategic plan. The Director of Community Care influences activities related to population health efforts and connected care across the continuum as it pertains to: Care Management/Care Coordination, Integrating Care Management/Population Health across the continuum in the Central Florida Division.

The value you’ll bring to the team:

  • Establishes collaborations for value-based lives between health management, care transformation, the clinically integrated network and diabetes institute
  • Oversight responsibilities for the screening, identifying, and resourcing social needs resulting in clear care coordination efforts by the Community Care clinical team
  • Oversight responsibilities for the development, deployment and monitoring of integrated cross continuum ambulatory pharmacy services, such as medication therapy management and medication reconciliation, behavioral social health services, and health related social needs.
  • Develops & Deploys Connected Care with focus on network integrity
  • Oversight responsibilities for the development, deployment and monitoring of the Health Coach Program within Community Care including establishing relationships and facilitating partnerships with Universities
  • Evaluates the health economics of Community Care as related to the efficiency, effectiveness, value and behaviors in the operations of the Community Care program that support Population Health efforts
  • Achieve Clinical Quality & Outcomes
  • Oversees operationalization of quality & utilization efforts with the Community Care program including the Medical Directors
  • Ongoing Program Development that addresses the opportunities and data drivers to improve quality, cost, and experience
  • Excel at Operational Performance
  • Ensures performance in areas of team, clinical, service, market and finance.
  • Creates and maintains efficient and effective processes to minimize cost, eliminate waste, and prevent provider and employee burn out.
Qualifications

The expertise and experiences you’ll need to succeed:

  • Master’s degree in a health-related field of study
  • Five years’ experience working in a health care related field
  • Five years’ experience in a healthcare or clinical management/leadership
  • Case management/adult chronic disease background preferred
  • Home Health or community experience preferred
