Job Summary
• Documents Social Work Assessments of individual’s needs, strengths, symptoms, behavior, and living skills for care planning
• Contacts family members and community providers to collect collateral information to develop a comprehensive assessment.
• Provides advanced clinical knowledge and skills to provide therapeutic interventions that will support patients in the stabilization of acute psychiatric symptoms and modulation of maladaptive behaviors so they may meet their treatment goals/objectives and move quickly to the next step on the continuum of care.
• Facilitates didactic and clinical groups.
• Conducts clinical 1:1 interventions to achieve goals and objectives as ordered on the treatment plan.
• Provides family counseling and psychoeducation.
• Interfaces with MD’s and other Treatment Team members (OT, Nursing, Pharmacy) re: clinical issues, development of safe intervention and coping skills plans, and recommendations for post-hospital care.
• Collaborates with Discharge Social Workers regarding pertinent discharge issues.
• Ensures timely, complete and quality documentation of initial assessments, Individualized Plans of Care (IPOCs), clinical 1:1 and group interventions, and follow-up information gathered during patients’ hospitalizations.
