The WE CARE Clinic located on the East St. Louis campus invites applicants for a full-time Health Education Coordinator position. The WE CARE Clinic provides primary patient care on the SIUE East St. Louis Higher Education Campus. Advanced Practice Nurses, a collaborating Physician, a licensed clinical Social Worker, certified health coaches and Pharmacist provide full services to diverse clients in the metro-east or other local communities. The Clinic serves patients of all ages, beginning with children age 2 or older.

The Health Education Coordinator will collaborate with community partners such as housing managers, church representatives, school nurses or administrators, and food pantry leaders to coordinate and implement chronic disease education (asthma) and other community education that is available to them free of charge through the clinic. The Health Education Coordinator will oversee the Asthma Trigger Assessment Program (ATAP). ATAP is a home visiting program that provides education and remediation for asthma in St. Clair and Madison Counties. The candidate will conduct asthma home visits and provide health coaching for community groups and individual clients about chronic illnesses such as asthma services available at the clinic. This grant-funded position educates the public about the Asthma Trigger Assessment Program (ATAP) which is in place to educate parents, families, children, and other individuals who might encounter people with asthma. The Coordinator will also meet with all clients identified by WE CARE FNP for potential home assessments, health education, smoking cessation, or other needs as identified and then facilitate health education, perform home visits for asthma triggers, conduct smoking cessation classes and individual sessions with the identified clients. Additionally, this position will represent the WE CARE Clinic at health fairs and community events. Other duties and responsibilities may be assigned based on the needs and goals of the program. Due to the professional responsibilities of this position, occasional evening and weekend hours may be required. Some travel within community is required.

Minimum Acceptable Qualifications:

1. Bachelor’s degree (120 hours) in public, mental, and/or medical health education, social work, psychology, student personnel/affairs, nursing, education or communications.
2. Two (2) years of conducting professional presentations, seminars or closely related programs and/or providing health education experience.

Please Note: An applicant with a Master’s degree in an area of health education qualifies with one year of professional communications and/or health education experience

Please Note: Possession of a Certified Health Education Specialist and/or Master Certified Health Education Specialist certification meets the requirements of #1 and #2 above.

Preferred Qualifications:

-Ability to network within the local and community, including key contacts for local housing authorities, local and county government.
-Master's in health related field preferred; demonstrated ability to work with individuals and potential clientele.
-At least 21 years of age or older.
-Able to meet with clients on a weekly basis as needed, including occasional evenings, and Saturdays as required.
-Complete home assessments for asthma triggers independently.
-Ability to work as a part of a team, requires minimal supervision.
-Have a vehicle, Illinois driver’s license, and auto insurance to complete at home visits.
-Interest in positively impacting adults, children, and youth through providing education and home asthma trigger assessment program services and smoking cessation.
-Must complete CCP certification within 6 months after hire.
-Requires excellent communication and writing skills.
