Harborview Medical Center’s (HMC) Pioneer Square/Downtown Programs - has an outstanding opportunity for an 60% FTE (24 hours per week), Day shift Registered Nurse (RN2). Shifts are Mon, Wed, Fri 1100-1930.

Harborview Medical Center is looking to hire a Registered Nurse, RN; to work in our Pioneer Square Downtown programs which focus on Harborview's mission population to address health equity issues by increasing access to care in our unique primary care settings, homeless shelters, housing programs or community settings for people without housing or vulnerable housed. This position will be sited in one of our Healthcare in Housing sites; 1811 Eastlake. Must be skilled at taking care of people with comorbid conditions including substance use/abuse, mental illness and complex physical health conditions with the ability work collaboratively with an outside agency and transitions of care from ED or hospital discharge.

RESPONSIBILITIES:
Provide triage and direct patient care including: wound care, blood draws, point of care testing, vitals, immunizations, medication management and nursing assessments.
Outreach and engaging the residents of the housing program with the intention of engaging the residents in their healthcare.
Record medical history, triage documentation, nursing visit and completion of orders.
Provide health education, transitions of care and support chronic disease management in conjunction with primary care provider and community health worker.
Support patients utilization of community and social resources.

SCHEDULE: 60% FTE (24 hours per week), Day shift, 3 days a week. Mon, Wed, Fri 1100-1930

LOCATION Pioneer Square/Downtown Programs – 1811 Eastlake, Seattle, WA

DESIRED:
Ability to collaborate with outside agencies to facilitate primary care.
Experience working with people who are homeless and economically disadvantaged
Experience working with diverse populations including limited English proficiency and immigrants

LEGAL REQUIREMENTS:
Washington State RN license required by start date.
Degree in Nursing from an accredited school of nursing.
