TDOH - Statewide RN Community Nurse Navigator - 2200000091 TDOH - RN Community Nurse NavigatorSalary: $72,800These positions are statewide and will have a reporting center that will be located at the various extension offices throughout the state.Here are the areas we are still needing nurses in:Southeast Region: 2 positions availableUpper Cumberland region: 1 positions availableSouth Central Region: 2 positions availableMid Cumberland Region: 0 positions availableWest Region: 1 positions availableThis position supports the College of Nursing’s TDOH Community Nurse Navigator Project to increase the immunization status of children and older adults in Tennessee. This position will develop and coordinate all activities to improve immunization rates in the counties assigned. The registered nurse will perform at the top of his/her training and education..Main Job Responsibilities: Support the TDOH Community Nurse Navigator Project through professional nursing community health assessment:
- Through the training provided, acquire in-depth knowledge regarding immunizations and working with communitiesConduct baseline and follow up assessment of each of the assigned counties of the barriers to immunizations, immunization rates, and places to obtain immunizations for children and older adultsIdentify priority counties to begin interventions based on data obtained
- Provide feedback to project director, evaluator, Dean and Associate Dean regarding activities and progress in immunization ratesDevelop and implement immunization outreach programs for counties in conjunction with extension agentsWork collaboratively with community agencies to identify and implement interventions that will improve immunization rates in children and older adults Work closely with the UT System Nursing programs to identify and implement interventions together to improve immunization rates in children and older adults. Supervise students as needed
- Keep accurate record and gather data as requested by the projectContribute to reports, manuscripts and presentations about the projectActively participate in regular grant meetings
Qualifications
Required Education & Experience:
- Bachelor's degree; RN license; at least 1 year of experience
- Nursing care experience preferred
- Knowledge of nursing care functioning in a RN roleCompetent with technology and EHRAbility to multi-task and prioritizeGood communication skills both written and orallyAble to work well in a team environment
