Sr. Clinical Social Worker
The Johns Hopkins Pediatric, Adolescent, & Young Adult HIV/AIDS program at the Johns Hopkins Hospital, a JCAHO accredited health care institution, provides comprehensive health and social support services to infants, children, youth, and young adults through the age of 25 years living with or affected by HIV/AIDS. Our mission is to assure the best quality of life for children, adolescents, young adults and families living with HIV by providing state-of-the-art comprehensive medical care in the context of sensitive and compassionate support services, without regard to ability to pay for these services.

The Division of Adolescent Medicine seeks a Sr. Clinical Social Worker to work under limited supervision, to provide high-quality comprehensive HIV mental health care, psychosocial support and specialized HIV medical case management in order to promote the mission, goals, and strategic vision of the Johns Hopkins Pediatric, Adolescent, and Young Adult HIV/AIDS Program. The Sr. Social Worker is experienced and knowledgeable of HIV/AIDS and treatments options and the unique psychosocial impacts of the diagnosis and treatment on our youth patients and their families.

Provide ongoing social work and medical case management services, psychosocial support groups, crisis intervention, and mental health counseling and treatment planning to youth and young adults infected and affected by HIV/AIDS. Utilizing in-depth knowledge, skills and expertise of working with this special and vulnerable population they will work to reduce health disparities and improve health outcomes of people living with HIV (PLWH), by identifying and removing barriers to medical care, facilitating continuous engagement in primary medical care, and supporting adherence to treatment regimens through the provision of individualized treatment planning and on-site supportive medical case management.

The Sr. Clinical Social Worker is often one of the initial clinical contacts for the patients, a crucial member of the interdisciplinary team, and must be able to prioritize work and patient's needs, manage stress, display critical thinking and decision-making skills, possess excellent communication skills, and display a patient first work ethic.

Specific Duties and Responsibilities:

• Provide daily, specialized social work services to children, youth, and young adults who have been diagnosed with HIV/AIDS, who are affected by HIV/AIDS, or who are at risk of acquiring HIV; including, psychosocial assessments, supportive counseling, care coordination, case management, crisis intervention, education, outreach, and resource referral for patients and their families.
• Conducts the initial psychosocial assessment, to address prioritized needs of patient and develops the initial plan of care for patient based upon the needs identified in the psychosocial assessment,
• Provides ongoing social work and case management services as prescribed in plan of care. Makes referrals for additional internal and community resources as needed and assists patients in securing resources. Engages with internal and community resources to ensure patient is receiving necessary services.
• Independently or under supervision of psychiatrist, provides patients with comprehensive mental health assessment/s, develops and implements mental health care plans, episodic and ongoing mental health counseling, crisis intervention and referral for emergency treatment or inpatient admission.
• Initiate, plan, implement and oversee clinic psychosocial support groups based on patient need and interest.
• Assist older patients living with HIV in transitioning to adult medical and psychiatric care through the use of care coordination services, education, motivational interviewing, counseling, treatment planning, and collaboration with adult colleagues at The Bartlett Clinic, Bartlett Psychiatry Clinic and other Baltimore based Adult HIV Clinics.
• Maintain timely documentation on all contacts, with or on behalf of the patient, in JHH EMR EPIC and HRSA CAREWare database. Complete all documentation required under Ryan White Standards of Care and Conditions of Award. Responsible for the completion of Ryan White Work Plan Updates / Progress Reports, as required by the grantee, for applicable senior social worker funded services.
• Utilize in-depth knowledge and advanced analytic skills to initiate and participate in continuous quality management activities. Generate and review mental health/psychosocial clinical tracking data in order to monitor performance goals and effectively implement Ryan White program services and compliance to Standards of Care.
• Utilizes in-depth knowledge of HIV patient populations to facilitate collaborations with clinical investigators who seek to enroll patients into relevant research, including clinical trials.

Minimum Qualifications:

Master's degree in Social Work from a program accredited by the Council on Social Work Education or an equivalent organization approved by the Council on Social. Current Social Work Licensure in State of Maryland, LCSW-C, required. Minimum of 2 years of experience providing clinical social work services in a health care setting to clients/patients from culturally diverse backgrounds, who are living with HIV, and at- risk of HIV. Must maintain license throughout duration of employment.

Preferred Qualifications: Five years of experience.

Additional Knowledge, Skills, and Abilities:

• Knowledge of the clinical aspects and epidemiology of HIV infection and experience with, and understanding of, psychological, social and medical needs of people living with HIV.
• Have a strong commitment to improving the health and quality of life of people living with HIV/AIDS and the desire, ability and commitment to build rapport and provide services to children, adolescents, and young adults with complicated psychosocial backgrounds.
• Excellent communication and collaborative skills, Strong interpersonal and organizational skills. Leadership ability, culturally competent and sensitive to high-risk population; possess a non-judgmental attitude, strong advocacy skills.
• Demonstrated knowledge of HIV/AIDS services and resources in Maryland and Ryan White HRSA guidelines, along with an interest and ability to expand knowledge through training.
• Knowledge of HIV treatments options, prevention strategies for PLWH and high-risk individuals, and treatment adherence interventions.
• Knowledge of mental illness, substance abuse, health behaviors, and community resources.
• Experience and skilled with the provision of evidence based mental health therapies such as Motivational Interviewing, Cognitive Behavioral Therapy and Trauma Focused/ Informed CBT
• Ability to support patient -specific advocacy while upholding patient policies and procedures.
• Ability to build relationships and networks of collaborators inside and outside the institution to achieve project goals.

Physical Requirements:

• Primary care site, hospital or community based medical environment- fast paced, requiring flexibility and adaptability.
• Work is generally sedentary in nature; however, some standing, stooping, and bending are required.
• Work requires concentration and constant attention to accuracy and detail.
• Work requires cross-training and coverage of colleagues.
• Work requires daily use of a computer.
• Work requires regularly handling multiple client cases involving a variety of providers, situations, deadlines and multiple priorities.
• Work may require site (e.g., home or community-based) visits
• Work requires attendance at team meetings and periodic off site Ryan White Meetings.

This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
