- Conduct comprehensive, focused assessment of children and families with palliative care needs.
- Formulate, implement, monitor, evaluate, and modify palliative care plans and program activities with input from palliative care team and other collaborating care providers.
- Assist children and families to develop self-care capabilities relative to identify problems.
- Assist in all facets of care coordination for palliative care referrals.
- Engage in quality improvement in patent care services and department functions, including evaluating the effectives of the health care delivery system in meeting the health care needs of pediatric palliative care patients and families.
- Implement patient and family education in collaboration with the interdisciplinary team.
- Participate in pediatric palliative care research, including quality and performance improvement initiatives.
- Demonstrate accountability for evaluations of clinical practice, including adherence to organizations, state and national practice standards and codes.
- Instructor: $158,000 - $165,000
- Senior Instructor: $163,000 – 175,000
- Assistant Professor: $173,000 - $185,000
Qualifications
Minimum Qualifications:Instructor
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Minimum 1-3 years of acute care and/or outpatient experience.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Minimum 3-5 years of acute care and/or outpatient experience.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Minimum 5-8 years of acute care and/or outpatient experience.
- Palliative Care fellowship training highly recommended; consideration given for certificate/Master's program in Palliative Care.
- Must successfully pass a drug test through Children’s Hospital Colorado.
- Must be willing and able to pass a national criminal background check.
- Knowledge of a variety of management of common pediatric and adolescent medical conditions, in the inpatient/outpatient and home-based hospice settings.
- Skills in the effective delivery of healthcare via phone and telehealth services.
- Strong spirit of collaboration and skills in working with an interdisciplinary team.
- Wide spectrum of knowledge of pediatric disease, pediatric prognostication and pediatric pain/symptom management.
- Highly skilled in empathic communication.
- Experience and desire to educate on the principles of pediatric palliative care.
- Superior organizational and prioritization skills, problem solving, and collaboration skills are necessary.
- Expertise and training in history and physical examinations and determination of treatment plans necessary.
- Completion of pediatrics or med/peds residency.
- Current CO state prescriptive authority.
- Inpatient experience and well-newborn experience.
- Completion of a palliative care fellowship.
- Palliative Care Board certified or eligible for certification.
