The Clinical Program Coordinator III position working under the supervision of the Chief and Clinical Professor of the Child and Adolescent division of the Department of Psychiatry will work with the DOH (Department of Health) collaborative program. Duties will include therapy referrals for patients, obtain additional screeners/rating scales, patient/family contact, scheduling the appropriate appointment type determined during staffing, making referrals to behavioral health agencies for therapy and support services for hub clients in support of PCP’s (Primary Care Providers) (therapy CAT(Community Action Team). Assist in referrals for other support agencies such as CARD (Center Autism Related Disabilities), schools, social service agencies.
Program Support- Adherence to and timely submission of DOH (Department of Health) contract deliverables; outlined in the DOH contract which relate to Quality Initiatives and Behavioral Health data tracking and collection (both within internal database and sharePoint developed by DOH sub-contracted program evaluator), and submission of de-identified Behavioral Health care coordination report. All tasks are submitted on quarterly report. Data elements are captured across all cases on a real time basis. Responding to receipt of all referrals within two business days, work with the participating PCP practices to obtain missing/incomplete documentation, making referrals to behavioral health agencies for therapy and support services for HUB clients in support of PCP’s (therapy CAT, etc.), Assist in referrals for other support agencies (CARD, schools, social service agencies), assisting with obtaining biopsychosocial information with families, facilitate communication with PCP offices, staff and families, work on scheduling different types of consultation (working with providers and clinic staff) after staffing of consults, assistance to PCP’s in making referrals for psychological therapies and other behavioral health or community services.
Administrative – Work closely with the Division Chief and Lead Psychiatrist to support the DOH program, duties will include participating in the QI activities as required, document all activities in database, participate in weekly staffing of referrals, ensuring all required documentation is received and complete prior to including for weekly staffing, tracking services provided on databases, processing of referrals, ensuring proper documentation is captured and screening data accompanies each referral.
Community Liaison – Responsible for coordinating multidisciplinary clinical/administrative teams and maintaining liaison relationships with university organizations and mental health care resources.
Expected Salary:$70,000 - $75,000
Minimum Requirements:Bachelor's degree in an appropriate area and four years of relevant experience.
Preferred Qualifications:Licensed Clinical Social Worker or case manager
Working knowledge of Excel, Outlook and Microsoft word
