Case Manager will advocate to increase opportunities for individuals with Autism Spectrum Disorder and families raising individuals with Autism Spectrum Disorder. Case Manager will identify resources in the community and work with community agencies to integrate resources. The Case Manager will be responsible for following up on online inquiries for resources and make needed connections. This position will complete targeted and strategic outreach events to share details of Autism Lifeline Links and TAMUSA, and will maintain a cooperative working relationship with direct services agency, including regular contact and collaboration.

The Case Manager will support the family or individual to make informed choices regarding important aspects of life (such as therapy, medications, school, etc.) and will follow up based on needs to discuss progress, problems and plans. This position will maintains full, accurate and current documentation as required by policies and procedures.

The position is funded in whole through gift funds and is contingent upon available gift funding.

Roles and Responsibilities:

  • Assess strengths and needs that impact family functioning in order to identify resources

  • Collaborate with community members to make referrals to appropriate agencies and stay abreast of new resources and trends

  • Provide short-term crisis intervention and emotional support to help alleviate stressors

  • Respect the confidentiality of records and uses complete discretion when discussing information

  • Perform additional duties as assigned

Required Education and Experience:

  • Bachelor’s Degree in social work or related health and human services field

  • 5 Years of related experience.

Preferred Experience:

  • Experience in the delivery of services and supports to persons with Autism Spectrum Disorder, related conditions or similar disabilities

  • Experience with client databases and creating reports

  • Ability to speak and write in Spanish is preferred

  • Must be able to connect with a wide range of resources

  • Work as an independent member of a small and highly interdependent team

  • Good analytical skills with high attention to detail and accuracy

  • Ability to be flexible and adjust plans quickly to meet changing needs

  • Ability to take initiative, identify opportunities and implement change

  • Strong written and verbal communication skills

  • Experience working with culturally diverse populations

  • Preference given to data/evaluation-oriented individuals with highly effective facilitation skills

  • Preference given to candidates who have documented experience with communities of color
