The Marcus Institute for Brain Health (MIBH) is an interdisciplinary clinical care and research institute for Traumatic Brain Injury (TBI) and associated physical and psychological health conditions such as post-traumatic stress, depression, and anxiety. The MIBH is part of the University of Colorado Anschutz Medical Campus (CU AMC) which is the largest academic health center in the Rocky Mountain region and is located in Aurora, Colorado. Aurora is Colorado’s second largest and most diverse city, home to a significant population of refugees.   

The MIBH provides specialized care for people with mild to moderate TBI, who have persistent posttraumatic symptoms, with a primary focus on the provision of subspecialty clinical services for military Veterans of pre- and post-911 era, regardless of discharge status. The MIBH provides comprehensive interdisciplinary intensive outpatient evaluation and treatment integrating novel medical, and complementary-alternative treatment modalities. This position will work entirely within the MIBH but will be assigned to an appropriate academic home department within the CU AMC.

 

MIBH currently seeks a Social Worker or Registered Nurse who will work within a comprehensive interdisciplinary medical team to assess patient needs. The position will provide screening interviews to identify at-risk social and behavioral patients being seen at MIBH.  The position will coordinate referrals to resources within the community to further address medical, mental health, behavioral health, and social needs of the patient. When identifying at-risk patients, this position must be able to make critical decisions regarding the patients’ mental/physical health needs to include enhanced case management assistance. The position must adhere to the State of Colorado Code of Ethics for their respective discipline.

 

Duties include, but are not limited to:

 

  • Demonstrate the ability to provide appropriate care to Veterans, family members and caregivers.
  • Assist Participants with navigating the admission process and paperwork prior to arrival at MIBH.
  • Assist Participants with navigating the University campus and MIBH appointments while engaged in treatment at MIBH.
  • Build relationships with patients that foster early communication and clarity of patient treatment goals.
  • Conduct and document biopsychosocial assessment in UCH electronic medical record system, including billing as appropriate.
  • Complete required documentation and electronic data entry to support continuity of care, program evaluation, and outcome data. Ensure providers receive pertinent information related to patient in order to create individualized and effective IOP treatment plan.
  • Conduct patient orientation for each treatment cohort.
  • Plan and lead patient care conferences and clinical rounds and ensure a clear and deliverable message for patients
  • Collaborate with the clinical team to prepare recommendations for after-care needs.  Meet with patients and their families to assess ongoing needs and provide community referrals and resources as appropriate.
  • Collaborate with MIBH VA Liaison to assist with patient access to VA services as recommended by the clinical team.
  • Serve as a liaison between patients and the referred resources to ensure psychological and medical continuity of care after MIBH participation.  This includes collaboration with MIBH VA Liaison to assist with patient access to VA services as recommended by the clinical team.
  • Use appropriate screening criteria, knowledge, and clinical judgment, to assess high risk behavior, such as substance use and suicidality, as well as monitor ongoing Care Plans.
  • Create after-care plan with patients during IOP and follow up for one year post discharge.
  • Advocate on behalf of patients to ensure we are meeting their clinical goals and other personal needs while engaged in care at MIBH.
  • Collaborate with the MIBH Veteran Relations Coordinator to establish relationships with veteran service organizations, regional providers, and national community organizations serving as collaborative resources for the MIBH patients. 
  • Empower program participants to improve their quality of life and ensure an integrated approach to address ongoing medical and psychological health issues.
  • Establish strong relationships with university staff, providers, and community partners.
  • Participate in regularly scheduled clinical supervision, case consultations, case management and team meetings.
  • Represent the university and MIBH in a professional manner.
 

Knowledge, Skills, and Abilities:

 

The Case Manager must be licensed or be able to obtain a license in Colorado and have knowledge of human development and psychological theory as it relates to traumatic brain injuries, human behavior, addiction, and PTS. They should have an understanding of the traumatic brain injuries and its impact on physical and psychological states and relationships, and they should possess relationship-building skills across a diverse range of cultural settings.

    Salary and Benefits:

    The salary range (or hiring range) for this position has been established at:

    Instructor                           $65,000 - $75,000

    The salary of the finalist(s) selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training.

    Qualifications
    Minimum Qualifications:

    • Master’s Degree in Social Work and LCSW in Colorado (or the ability to obtain)
    • or Bachelor of Science Degree in Nursing
    • 2 years’ experience with case management
     

    Preferred Qualifications:

      If BSN, Case Management Certification (or ability to obtain)Master’s Degree in Nursing (if not LCSW) 2-3 years direct experience with mild to moderate Traumatic Brain Injuries Experience working with active-duty military and/or Veterans
