Health Information Clerk 2

The University of Tennessee, College of Veterinary Medicine, invites applications for the position of Health Information Clerk 2.  The primary purpose of this position is to perform clerical duties to maintain the function of the Health Information Unit by ensuring medical records are properly processed and maintained in accordance with established policies and legal guidelines and ensuring that clinicians (faculty, residents, and interns) and other healthcare providers comply with established standards for proper record completion. This position has the responsibility of performing release of information functions for referring veterinarians in accordance with established guidelines.  Under limited supervision, this position may make independent decisions based on documented policies and procedures.   

Typical Responsibilities:

Performs comprehensive chart assembly and ensures the accurate placement of all medical reports and documents and the accurate filing of loose medical records in more complex patient records. Performs complex analyses of patient records to determine level of completion and/or deficiency. Extracts data from record for completion of statistics and inputs data into computer. Verifies accuracy and validity of data entered. Scans and uploads documents. Complies with requests from referring veterinarians for copies of records. Maintains Release of Information Log. Maintains forms management inventory. Provides work direction to Health Information Clerk 1 if necessary.

Enters, corrects, and reads data regarding chart locations, chart deficiencies, registration of owners, episodes of care, and patient entries. Assists in maintaining an accurate and up to date Incomplete and Delinquent Record Management and Reporting system. Performs follow-up analysis of patient records returned by faculty, residents, interns, and healthcare providers, indicating whether or not chart deficiencies have been corrected.

Assists in verifying data for statistical reporting. Enters demographic information. Performs data review for accuracy and completeness. Assists with Forms management. Performs other related duties as required.



Qualifications
Minimum Qualifications:

High school diploma or equivalent.  Entry-level position. Must be at least 18 years of age.

 

Preferred Qualifications:

 

6 months of relevant experience in a medical records environment.
