Job Description

The department of Psychiatry within the School of Medicine is pleased to announce the opening of a part time/PRN RN Case Manager for our Bridge Program.

The Bridge Program markets and seeks referrals from school counselors, parents, or primary providers for children who need a psychiatric evaluation. Bridge provides a free evaluation of the child at school by a child psychiatrist with the parent or guardian present. The Bridge team then provides two or three follow up appointments at no cost to the family to discuss possible treatment options. Our Bridge Nursing team helps explain options to the parent or guardian and helps connect the family to treatment outside of the program. This position reports to the Manager of the Bridge program and works to provides necessary services to achieve the program goals and outcomes serving children. This position collaborates with the Child Psychiatrists working in the program and other staff on the project.

This is a fully grant funded position and program. The funding requires a Case Manager have an RN license.

Primary Responsibilities Include:

*Consult with schools and other community providers for student referrals;
*Coordinate with outreach counselors for completion of program forms and releases;
*Schedule appointments and attend school-based clinics with the program’s psychiatrists;
*Maintain regular contact to and from parents/caregivers, school staff, and other providers of care for each student-participant;
*Attend meetings with school staff and other school-based providers to collaborate, refer, and share students’ progress;
*Maintain data collection and assist in interim and end-of-year reporting

Salary

Salary Range: $26.90 - $33.09/hour

Grade: PAT-8A

Part Time hours during Monday through Friday 8am – 5pm

Minimum Qualifications A Bachelor's degree from an accredited School of Nursing or an equivalent combination of education and experience from which comparable knowledge and abilities can be obtained is necessary.

Two to four years of nursing experience in area of specialization is necessary.

Possession of a license to practice as a Registered Nurse in the State of Missouri is necessary.

Preferred Qualifications

Funding agency requires RN License for the position.
