Job Summary:

The Collaborative Care Therapist will work as a member of the multi-disciplinary health care team to provide primary and secondary collaborative care services to all eligible patients. The Collaborative Care Therapist provides patient screening/assessment, risk reduction education and counseling (i.e. sexual behaviors, substance abuse, mental health) and links the individual to appropriate providers, agencies and programs for mental health and substance abuse treatment assistance. Provides therapy in individual and group settings. Identifies available resources, informs the patient about available resources, educates the consumer about accessing primary medical care and specialty care services and prescription assistance services, coordinates referral services, assists with transportation to services and coordinates the overall process to see that services are timely, affordable and of high quality.

Must be able to travel, have dependable transportation; must be sensitive to rural communities, communities of color, gay/lesbian/bisexual communities, disenfranchised communities, and persons who have been or are substance abusing, incarcerated or homeless.

Environmental Hazards:
Possible exposure to hazardous material, hot and cold, blood borne pathogens.

Physical Requirements:
Ability to lift and carry 8-10 pounds, stoop, reach, stand, walk, finger, grasp, feel, talk, hear, see, and perform repetitive motions with or without reasonable accommodations.

Education & Experience

Position Qualifications:

Required:
- Minimum MS with licensure as LPC, or LCSW by State of Oklahoma
- Two (2) to Five (5) years professional experience
- Previous case management experience with HIV-infected and/or affected individuals

Preferred:
- Case Management certification
- Substance Abuse training/education with CADC certification
- Professional experience with substance abuse
- Professional experience with risk reduction patient education and/or harm reduction
