Nurse Clinician (Patient Navigator) - Women's Health - Full-Time Days - Durham

The RN in this role will actively coordinate the team-based management of high-risk medically complicated pregnant patients and specialty gynecologic care including pediatric and adolescent gynecology and menopause care in the inpatient and outpatient settings and support Duke Health and the Department of Obstetrics and Gynecology in the achievement of quality initiatives and reporting. This position will be 80% dedicated to care coordination of high-risk medically complicated pregnant patients under the care of the Division of Maternal Fetal Medicine (MFM) and 20% dedicated to supporting and developing our Pediatric and Adolescent Gynecology (PAG).

The overall goal of this position is to improve the health and decrease health disparities among medically complicated pregnant patients and improve the care coordination of pediatric and adolescent gynecologic care.

The high-risk obstetric and gynecologic care specialist will:

  • Manage the coordination of all cases referred to the Complex Maternal Surgery program, mostly consisting of care coordination for patients with placenta accreta spectrum between MFM, gynecologic oncology, interventional radiology, obstetric anesthesia and perioperative surgical scheduling.
  • Coordinate care of pregnant medically complicated patients between multiple sub-specialty consultants, such as cardiology, endocrinology, hepatology, oncology, etc. with MFM, and coordinate multidisciplinary conferences when necessary.
  • Attend inpatient huddles and work with referring providers to ensure adequate feedback and appropriate site and cadence for follow-up of medically complex obstetric patients after hospital discharge.
  • Help medically complicated pregnant patients navigate the transition from the hospital to the outpatient setting coordinating multiple specialist appointments.
  • Participate in growth of the MFM telehealth program via coordination with potential new collaborative partners.
  • Manage the coordination of patients referred to the PAG program, including care coordination with other services including adolescent medicine, pediatric surgery, pediatric urology and pediatric hematology as needed.
  • Provide education to patients, and for PAG patients their families, about their condition, follow-up needs, and delivery planning.
  • Managing health care records, including ensuring adequate incoming referral records and communication back to referring provider.
  • Managing follow up visits and ensuring continuation of care and linkage to community resources for these populations as needed.
  • Participate in Departmental and Hospital committees and meetings to facilitate improvement in patient care and development of goals, philosophies, and objectives for a specialized patient population.
  • Participate in all applicable regulatory reporting and quality initiatives.

Work Hours:

Typically Monday - Friday, full-time.

Minimum qualifications:

  • BSN required
  • Three years of experience in High-Risk Obstetric nursing required
  • North Carolina RN Licensure
