The East Carolina University Community School involves cross campus collaboration to provide the students a spectrum of support and services to comprehensively address their academic, social-emotional, physical, and family/community needs. The school will include one class per grade level for pre-kindergarten through 5th grade. The community school involves an integrated partnership with Pitt County Schools and will be housed within a local elementary school.Job Duties :
The Integrated Health Collaborative of the ECU Community School & Pitt County Schools seeks a Director of Integrated Health to build, implement, and sustain an integrated care model that will serve children, faculty, staff and families from the Community School and South Greenville community.
As the Director of Integrated Health within the East Carolina University Community School, this person will construct a model of integrated care in collaboration with a team of physical, mental, social, and behavioral health service providers within the school and in tandem with the student services and the leadership at South Greenville Elementary. The Director’s primary role is to implement a sustainable model of health care that can improve the health and well-being of scholars, teachers, and their families through collaborative care within the school and through collaboration with community providers and partners. The Director will develop a clinical and operational model of integrated care that maximizes efficient and effective training for school based providers to deliver healthcare services. The Director will also create and evaluate the documentation for health assessments, treatments, outcomes, and referrals; family based interventions; follow up protocols; and services that enhance well-being for teachers and staff in the school environment.
—Connecting the Integrated Health collaborative from ECUCS and South Greenville Elementary with external community providers and partners, by:
—Attending student service trainings and system wide meetings on behalf of the ECU Community School and South Greenville Elementary.
—Building relationships with current and potential community partners and providers.
—Seeking out peer to peer support with other integrated care navigators/directors to stay on the front line of innovation in school based services and integrated care delivery models.
Implementing and Overseeing Integrated Care Protocols, by:
—Approving assessments and screeners indicated for diverse youth and families served by the ECU Community School and South Greenville Elementary.
—Determining how to offer services on-site that attend to scholars’ (and their family’s) diverse health needs and creating practices that encourage collaboration with community providers and partners.
—Creating and reviewing templates for care/treatment plans that will be used by all Community School and South Greenville Elementary providers who extend healthcare services to scholars and their families.
—Serving as a liaison between providers/community partners and scholar families when resolving concerns related to facility policies and procedures or patient care.
—Developing a structure for referral processes, including release of information and follow up documentation.
—Ensuring that all clinical and operational procedures are functioning in an ethical and systematic manner.
—Overseeing the documentation portal and ethical maintenance of records.
—Confirming HIPAA and FERPA compliance.
—Performing crisis intervention as necessary.
Management and Administrative Oversight of Interns and Graduate Assistants, by:
—Hiring and reviewing all graduate assistants in collaboration with aligned ECU Community School and South Greenville Elementary service providers
—Overseeing the work and scheduling of graduate assistants drawn from behavioral health programs who are responsible for intakes, assessments, service provision, referrals
—Delivering training to service providers and interns on the Integrated Care model deployed at the ECU Community School and South Greenville Elementary.
Enhances Employee Collaboratives, by:
—Facilitating weekly supervision meetings with the integrated care team to ensure continuity in model delivery.
—Communicating regularly with the Community School and South Greenville Elementary Principals to ensure continuity in the model between the schools.
—Offering employee support (i.e., for teachers and staff) to enhance retention and well-being.
Facilitates research and accountability related to health collaborative, by:
—Understanding and submitting any necessary school based reports requested by the Principal(s) or in accordance with state mandates.
—Tracking scholar health outcomes.
—Tracking the fidelity and success of the integrated care model.
The individual should be: self-directed; organized with attention to detail; innovation and outcome oriented; technologically proficient across multiple platforms; possess strong interpersonal relationship skills including collaboration with diverse individuals; highly motivated and be willing to be receptive to innovative practices/procedures; willing to take the initiative to learn the duties/responsibilities/expectations of the work and operations of the Community School.
Contingent upon availability of funding.Minimum Education/Experience :
Master’s degree in Mental Health Field, Counselor Education, Social Work or Nursing. The candidate must possess clinical and operational knowledge of best practices in integrated care. Applicant must have oral and written communication skills; and evidence of skill in working with school and health systems. Qualifying degrees must be conferred and received from appropriately accredited institutions.
Evidence of interpersonal relationship skills including collaboration with diverse individuals.License or Certification Required by Statute or Regulation :
Current NC Clinical License (active and in good standing).Preferred Experience, Skills, Training/Education :
Preference will be given to candidates who have working knowledge of child welfare, developmental disabilities, mental health and/or primary healthcare fields. Additional preferences would include experience using Excel and electronic health care systems to review and manage data, service coordination and information, linkages, and referrals for community-based services.Special Instructions to Applicant :
