General Summary

The Case Manager coordinates campus support systems and resources to implement a plan of care for students in need, and oversees the case management program for sensitive student problems. Serving as a member of the Student Intervention Team (SIT), the Case Manager assists the SIT Chair with overall case management by triage referrals, coordinating interventions, tracking students' completion of recommendations. This person works closely with other campus resource providers, clinicians, student affairs and academic units, and community support systems in coordinating a response that balances the needs of individual students with the safety and welfare of the University community.

Principal Duties and Responsibilities

Coordinate the case management program for sensitive student concerns, which includes but is not limited to; referrals to community agencies, entitlement programs, and off campus resources; to ensure continuity of care and access to support.

Provide timely disposition, management and resolution of assigned cases referred to the Dean of Students Office.

Triage referrals, schedule and coordinate interventions, assist in care plan development for selected students. Follow up with students to ensure compliance with recommendations.

Identify and disseminate the network of on and off campus services to assist students as appropriate.

Provide timely communication with the SIT members and other university administrators regarding students of concern and update case progression in the electronic database.

Design and implement Dean of Students Office outreach efforts targeting students, faculty, and staff, including presentation and marketing materials.

Assist with resolution and prevention of both academic and non-academic concerns.

Collaborate and establish partnerships with designated university departments to address concerns holistically and advocate for support.

Educate the campus community on subject matters within the Dean of Students Office.

Respond to walk-ins and phone calls related to students to assess their immediate or emergency needs, and provide crisis consultation services as needed. Provide referrals to appropriate health and social service agencies for additional services as required.

Make reports, investigate concerns, evaluate, and present cases to SIT team.

Serve as a point of contact and provide consultation for campus community members who are seeking guidance and advice about our services for students who may be experiencing distress; also communicate possible intervention steps and procedures.

Manage all data, functions and information in the Advocate database related to students of concern and maintain adherence to guidelines for disclosure and access.

Obtain and review updates on distressed students and facilitate the flow of information between appropriate offices.

Analyze, collect participation data, and report out on program outcomes to leadership.

Qualifications

Knowledge and skills in student development and case management typically acquired through the completion of a Bachelor's Degree in social work, counseling, psychology, college student personnel, higher education and student affairs, or a related field is required. A Master's Degree is preferred.

A minimum of three (3) years professional experience in student services/case management work requiring working with sensitive matters is required.

Knowledge and understanding of applicable laws (FERPA, HIPAA, ADA, Title IX), regulations, and policies within assigned unit and how they impact University operations and the student experience is required.

Proficiency in recognizing, problem-solving, and responding flexibly to highly sensitive situations as they arise with sound judgment and confidentiality.

Well-developed interpersonal skills with excellent written and verbal communication skills, including the ability to give group presentations.

Demonstrated success in working with diverse constituencies, both domestic and international, in a university setting is preferred.
