The Transitions of Care Enterprise Director/UKHC is responsible for strategic direction and operational oversight for care transitions along the care continuum with specific focus on length of stay and transition to the post-acute care environment. Collaborates with external clinical referral sources and payer sources to amend standard work and care models to support and facilitate value-centered, progressive and seamless care transitions. The Transitions of Care Enterprise Director will hold as a priority to continuously improve care transitions in order to improve health outcomes of post-acute hospitalization and reduce overall costs of care. Unifies and aligns resources and standard work within the UK HealthCare system and the community to facilitate a consistent approach to safe transitions from the short-term acute care hospital to the post-acute environment including inpatient rehabilitation setting, skilled nursing facility, home with home health, long term acute care hospital, hospice and swing beds within acute care facilities.Skills / Knowledge / Abilities:
The ideal candidate will have knowledge in care transitions with an emphasis on quality, safety, and value-driven care and a strong understanding of business acumen. We are seeking an effective leader who is innovative, results-oriented, customer-focused and a strong communicator. Must have experience with MS Windows, Excel.Does this position have supervisory responsibilities? :
