Job Summary:

This position is based the Infectious Disease Group Practice (IDGP) at the University of Colorado Hospital. The Linkage and Retention Coordinator provides supportive services to facilitate linkage, access to, and retention in care for patients in the IDGP; a busy, fast-paced clinic with a large caseload of patients.

Specific Responsibilities

  • Work as part of the multidisciplinary patient support and engagement in care teams; including participating in team meetings.
  • Conduct an intake/psychosocial assessment to identify needs and barriers to care.
  • Provides case management and psychosocial support to patients in addressing identified barriers and obtaining needed services until successful linkage or reengagement in care.
  • Support care coordination activities; including assisting patients with referrals to other support services; including social work, medical case management, mental health, substance use, and adherence counseling when appropriate.
  • Coordinate with the University of Colorado Hospital emergency department HIV testing program to facilitate timely linkage to care services to the IDGP.
  • Provide linkage services to newly diagnosed or new to care PLWH to facilitate entry into HIV medical care.
  • Provide patient specific health education to ensure that new patients are able to navigate the HIV system of care.
  • Coordinate with care team members to identify patients who are at risk for or have fallen out of care and support their re-engagement.
  • Provide outreach for patients who are at risk for or have dropped out of care.  
  • Work with patients to identify and address potential and/or existing barriers to engagement and retention in medical care.
  • Networks with community agencies and organizations in order to establish referral linkages.
  • Assure that clients are properly transitioned out of linkage to care and retention in care services.
  • Proper documentation of all linkage and retention in care activities.
  • Enters and maintains accurate data collection, enrollment, tracking, clinical documentation, and reporting forms.
  • Participate in clinic quality improvement activities as appropriate.
  • Other duties as assigned.  
Salary and Benefits:

The salary range (or hiring range) has been established at $50,000 to $60,000.  This position is not eligible for overtime compensation.

The salary of the finalist(s) selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training.

The above salary range (or hiring range) represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting.

Qualifications
Minimum Qualifications:

  • Bachelor’s degree in social work, public health or other related field.
Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications:

  • Experience working in HIV/AIDS and LGBTQ related activities.
Competencies / Knowledge, Skills and Abilities:

  • Experience with HIV and LGBTQ care issues.
  • Experience with conducting assessments.  
  • Case management experience helpful.
  • Ability to work independently and as part of team.
  • Excellent communication skills.
  • Flexibility, multi-tasking.
  • Sensitive to issues of diversity.
