Summary

The Patient Representative is charged with assisting their assigned primary care physicians in growing and maintaining full patient panels for comprehensive primary care, managing clinic schedules, communicating with patients to facilitate timely planned follow-up appointments, re-engaging with patients who miss planned appointments, facilitating referrals for specialty consultation, procedures and diagnostic studies, and providing access for acute care. The Patient Representative assists the primary care physician in all aspects of follow up to meet appropriate chronic disease and preventive care metrics for quality and participates in quality improvement and metrics reporting for the assigned physicians.

Required Qualifications

• High school diploma or equivalent
• Two years of related work experience

Preferred Qualifications

• Bachelor's degree
• 3 years work experience in clinic or administrative setting
• Proficient in the review of medical records and other patient care information
• Knowledge of Medicaid, Medicare, and other insurance precerticification procedures and requirements
• Knowledge of healthcare reform/policy
• Basic understanding of health, prevention, and disease systems
