Position Summary:

The Supervisor of Case Management is responsible for assisting with the alignment of case management competencies and metrics for assigned area. The CM supervisor will provide the day-to-day frontline supervision of assigned case managers, ensuring high-level assessment and reassessment to assure that each patient has a plan of care that is focused on high quality, individualized, safe and cost-effective transitions of care. Ensures that the documentation from each case manager in the medical record is complete reflecting the patient's true severity of illness.

Additional responsibilities will include the daily assignment, assistance with difficult discharges, scheduling and orientation of new staff. Serves as a role model for excellent service by training and reinforcing behaviors and standards among the staff that lead to positive interactions with patients and timely resolution of patient complaints. Promotes collegiality and teamwork by serving as a liaison between assigned units and other departments or disciplines across the Medical Center with a focus on patient throughput and decreasing length of stay.

Annual Rate $81,400 $133,500 $185,500

Required Qualifications:

  • Minimum 3 years of Case Management experience
  • Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload
  • Demonstrated leadership skills/experience
  • Demonstrated leadership, interpersonal skills, and ability to communicate with varied levels of customer audiences
  • Familiarity of Joint Commission, CMS, CDPH and CCS requirements
  • Ability to establish and maintain effective working relationships across the Health System
  • Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties
  • Must demonstrate customer service skills appropriate to the job
  • Demonstrated experience and ability to build effective working relationships and to represent the department effectively in order to accomplish goals
  • Ability to facilitate and lead interdisciplinary rounds
  • All external candidates must have a Bachelor of Science in Nursing
  • Computer literacy and proficiency
  • Bachelors' degree in relevant field or the equivalent combination of education and experience
  • Current California RN license
  • Excellent written and verbal communication skills in English
  • Must possess complete understanding of acuity levels for specific patient population


Licenses and Certifications:

  • Registered Nurse(Required)


Preferred Qualifications:
  • CCM, ACM or Case Management credentialing
  • Knowledge of University policies and procedures
  • Bilingual skills
  • Knowledge of California Children's Services benefits authorization process (CCS)
