•Hybrid home- and office-based position with productivity and quality expectations. Work requires access to reliable high-speed internet and a private home-office environment.
•Work will involve intermittent travel to professional locations to meet with provider teams and/or patients and families.
•Must reside within 90-minute commute of Fortify Children’s Health location for continuing education, operations meetings, technical support.
•Proficiency with computer skills including Microsoft Office-based programs, database navigation, EHR navigation and documentation required.
•Work includes extended periods of sedentary activity including talking on the telephone and using the computer.

The Fortify Cares Licensed Care Manager has the opportunity to leverage clinical expertise in caring for Virginia’s most vulnerable children and their families. This role will be responsible for building respectful rapport and trust with patients, families, providers and community partners; completing comprehensive assessments of physical and psychosocial needs; identifying appropriate healthcare and community resources; collaborating with families developing an individualized plan of care; and supporting and monitoring progress toward goals.

  • Program development and refinement in support of the Fortify Children’s Health mission and Fortify Cares priorities

  • HIPAA-compliant documentation

  • Comprehensive and continuous assessment and of patient and family including medical, social, educational and personal history

  • Identification of and access to community resources to optimize effective and efficient care outcomes in collaboration with the multidisciplinary team

  • Advocating for patients, family members in conjunction with the healthcare team and Fortify team members;

  • Patient-centered care planning that results in development, implementation and ongoing evaluation of an individualized plan of care that is agreed-upon by the patient and family and communicated between care providers while ensuring patients and family members are actively informed of and participating in the care process in conjunction with the care team members

  • Ability to document and communicate in compliance with HIPAA and other regulatory guidelines

  • Education and support for patients and caregivers

  • Ongoing Identification and Referral to appropriate community resources based on assessment of needs

  • Consistent documentation that meets care, regulatory and accreditation requirements

  • In addition to the above job responsibilities, other duties may be assigned.

Position Compensation Range: $60,236.80 - $96,387.20 Annual

MINIMUM REQUIREMENTS

Education: Bachelor's degree.

Experience: 2 years clinical practice experience, 2 years care management experience, CCM required within 2 years. Proficiency with computer skills including Microsoft Office-based programs, database navigation, EHR navigation and documentation required.

Licensure: Licensed Clinical Social Worker or Bachelor’s-prepared Registered Nurse with current, unrestricted Virginia license.
