8:00 am – 4:30 pm

Job Summary:

The LPN is a member of the multidisciplinary ambulatory team who is responsible for patient care, patient satisfaction, and clinical outcomes. The LPN will provide telephonic care to patients transitioning from a hospitalization or emergency room visit. The majority of this work will be completed off campus as a work from home position.

Skills / Knowledge / Abilities:

Triage experience is a plus.

Associate degree / professional license.
