Job Summary:

-Establish and maintain positive relationships dialysis centers.
Will work with dialysis center staff and physicians to provide education, information, and assistance to patients.
-Meet with patients to provide education, assistance, and information. Will discuss transplant experience and assist
patients in understanding transplant process, medications, wait listing, living donors, and post transplant care.
Obtain demographic and clinic information on new patients
referred from the Louisville area. Input patient information into
OTTR (as needed) for patient updates and record keeping on referrals. Partner with other staff support to route new referral
information and/or issues to transplant team. Prepare and provide education materials for patients.

Skills / Knowledge / Abilities:

Ability to be patient centered and proactive. Needs to be able to listen, and have attention to detail. Have advanced working knowledge of the kidney transplant and evaluation process.
