Job Summary QUALIFICATIONS

• Graduate of an accredited School of Nursing

• Licensed as R.N. in the State of Georgia.

• Maintain current BCLS certification

• Prefer ACLS certification

• Experience with Hospice program

• Supervisory Experience

• Minimum of three years nursing experience with at least one year which must be a Hospice related position.

• Previous experience in Hospice program development and/or management preferred.

• Good organizational and decision-making skills.

GENERAL DUTIES

Under minimal supervision functions as the coordinator and is responsible for the Palliative Care Program delivering patient care through the nursing process of assessment, planning, implementation and evaluation. Also, directs and guides patient teaching, staff education and coordinates multidisciplinary treatment team meetings.

RESPONSIBILITIES:

• Maintains professional attitude at all times.

• Tactful and diplomatic at all times; functions as a role model for staff

• Supervises licensed staff to include RN's, LPN's HST's and clerks.

• Interrelates and works effectively with patients/inmates, medical and nursing staff, security staff, counseling, social worker, chaplaincy, administrative staff and support staff, i.e. pharmacy, laboratory, etc.

• Assesses physical, psychological and social dimensions of patients within the health care unit and, as necessary, in the housing units.

• Uses problem oriented medical records system to chart using SOAP format and nursing protocols.

• Applies knowledge of available resources in obtaining health history and in the assessment process.

• Provides a written assessment of patient on admission during confinement and on discharge in accordance with SOP.

• Summarizes assessment data and implements a triage decision that is documented and communicated to other patient care providers.

• Maintains confidentiality of medical records.

• Utilizes referral process to appropriate providers as necessary.

• Applies knowledge and utilizes available resources in planning admission and discharge care and patient teaching.

• Consults, as necessary, with multidisciplinary team on patient care/treatment plans.

• Implements individual treatment program in the health care unit and/or housing units.

• Uses proper techniques, procedures and safety precautions to meet the individual needs of the patient.

• Administers medication according to proper techniques and procedures, including IV therapy and all other approved routes of administration.

•Utilizes pharmacology knowledge and available resource to include drug reaction and overdose in administering medications.

• Implements treatment plans as ordered by the physician or appropriate therapist.

• Implements nutrition and therapeutic diet plan as ordered by the physician.

• Follows facility procedures for obtaining therapeutic diets and monitors diet compliance.

• Communicates information to other nursing staff, physicians, medical unit supervisory personnel, correctional officers and department heads.

• Transcribes orders in accordance with SOP.

• Completes required shift report before leaving the unit.

• Complies with professional dress code.

• Reports to work as scheduled. Follows GCHC policies for leave.

• Keep assigned area clean and restock area as needed.

• Maintains compliance with all mandated logs and checks for medication, syringe, tools and controlled medication accountability. Does not leave unit until counts clear and logs are signed.Notifies supervisor of inaccurate counts.

• Ensures that prescribed medications are administered as ordered using the formulary. Documents administration of medication on the MAR in accordance with SOP at the time of administration.

• Properly disposes of all needles/syringes in accordance with Infection Control policies and procedures.

• Prepares regularly scheduled reports and special reports as necessary/directed.

• Participates in in-service education and training of staff.

• Provide patient education and counseling in treatment plans.

• Works with staff development coordinator in providing and coordinating training to staff related to the Palliative Care Program.

• Coordinates, schedules and functions as a Team Leader for the Palliative Care Treatment Team Meetings which must occur at a minimum of monthly and more often as needed.

• Takes and maintains minutes to the Palliative Care Treatment Team Meetings.

• Works with GCHC reprieve coordinator and ASMP medical director in identifying those patients eligible for reprieve.

• Works with ASMP social worker on discharge process and placement in community based hospice program.

• Fills in as scheduled when staff shortage arise.

•Responds to emergencies.

• Crossed trained as house supervisor and shares in supervisor call rotation.

• Performs nursing duties as assigned to meet nursing needs in the facility, this may include rotations and weekend duties.

•Performs CQI activities as directed to include audit preparation.

• Performs all other duties and responsibilities as designated by supervisor.
