Responsibilities

Compliance with Patient/Family-Centered Care standards through the following:

  • Demonstrates dignity and respect for patient and family for patient and family knowledge, values, beliefs, and cultural background in the planning and delivery of care.
  • Provides information sharing, ensuring patients and families receive timely, complete, and accurate information in order to effectively participate in care and decision-making.
  • Encourages patient and family participation in care and decision-making at the level they choose.
  • Promotes collaboration with patients and families in policy and program development, implementation, and evaluation in health care facility design, professional education, and delivery of care.

Education / Development:

  • Serves as a preceptor in the orientation of new SN/SSN or other patient care staff
  • Takes initiative to address issues in need of correction and makes suggestions for improvement
  • Produces acceptable work volume and effectively meets established deadlines, including annual education and performance evaluation
  • Expands knowledge of cardiac diagnostic catheterization and interventional procedures, and equipment involved to perform procedures
  • Complete mandatory compliance training within assigned timeframe

Delivery of Care:

  • Provides input into the plan of care by reporting pertinent information involving patient/family to the appropriate individuals (i.e. charge nurses, physicians or APPs in outpatient areas)
  • Provides report to receiving unit upon admit from clinic
  • Performs safe medication practices
  • Demonstrates a level of self-management to include working effectively with direction provided and independently while producing quality work in a timely fashion
  • Evaluates care as demonstrated by the ability to perform nursing actions that demonstrate accountability; reflecting knowledge of evidence based recommendations for practice
  • Complies with regulatory standards; such as labeling medications when assisting with Echo procedures or administering medications in clinic
  • Other duties as assigned

Communication:

  • Listens to patient and family concerns and offers help
  • Utilizes resources effectively for non-English speaking/hearing impaired patients/families
  • Reports equipment and computer failure to the charge nurse promptly
  • Perform patient/family teaching
  • Communicates collaboratively, using effective communication techniques with all members of the care team

Documentation:

  • Maintains accurate ambulatory intake information and promptly communicates abnormal vital signs or changes in patient condition to the appropriate care team members
  • Accurately documents care provided, supplies used, and procedures performed in a timely fashion
  • Provides timely verification of electronic date in the EMR
  • Encompasses all written and/or electronic entries reflecting all aspects of patient care communicated, planned, recommended, or given to that patient

Environment of Care:

  • Maintains a clean work environment to include all computers and equipment in the work area
  • Ensures patient safety, including reporting patient safety issues immediately
  • Maintains a clean and safe environment while observing infection control policies and procedures, proper hand-washing techniques and frequency, appropriate attire and use of universal and transmissions based precautions
  • Maintains a safe work environment to include proper management of sharps and biological/chemical hazardous materials
  • Follows hospital policy to appropriately identify patients prior to treatment or procedure
  • Ensures patient and employee safety related to use of equipment and technology within the work environment (to encompass cleaning, appropriate use and maintenance)

The Nurse Managed Care Center (NMCC) Registered Nurse is a professional registered nurse responsible for the nursing care of individual patients as well as populations of patients in the Nurse Managed Care Clinic(s). Care is provided through the use of the nursing process in a manner that is safe, provides high quality care and optimal outcomes while being cost effective. The NMCC RN performs assessments in conjunction with an Advanced Practice Registered Nurse to establish, implement, and evaluate a patient/family individualized plan of care. It is the NMCC RN’s responsibility to provide education and reinforce explanations during the course of the visit and at time of discharge from clinic. The NMCC RN is also responsible for participating in management of patient specific follow-up care (i.e. test results) as well as the care and management of populations of patients that are served by the NMCC. The NMCC RN participates in on-going departmental and organizational quality improvement initiatives; meets continuing education requirements; participates in student/employee orientation/teaching activities; upholds College of Nursing mission, vision, values, philosophy, goals and objectives; demonstrates growth and development through involvement in professional organization; and is committed to the review and integration of best evidence-based practices identified in the professional literature. Administrative responsibilities may include participation in research studies, participation in the administration of patient/family satisfaction surveys, and participation in the review of NMCC data.
