In addition to high quality primary care providers and staff, the expanded team includes licensed clinical social workers and peer support specialists who offer integrated behavioral health services including: health and wellness classes, one-on-one health coaching, behavioral health support, addiction recovery support, tobacco cessation counseling, advance care planning, intensive case management, and behavioral health care coordination.
The clinic’s primary care team also offers office-based medication assisted treatment (MAT) to individuals with substance use disorder; provides medical co-management to patients on the Wakebrook residential and inpatient psychiatric treatment units; and offers medical consultation to patients being evaluated at the Wakebrook Crisis and Assessment psychiatric emergency service.
