Position Summary:

The incumbent is responsible for the clinical aspect of patient care including phone and walk-in triage. Assists with procedures, patient education, outpatient and post hospital discharge follow-up including coordinating home care, hospice, equipment, physical and occupational therapy. The nurse incorporates the psycho-motor and age appropriate cognitive skills of the patient/family/significant other utilizing knowledge of patient assessment, planning, intervention and evaluation.

Required Qualifications:

    Ability to establish and maintain effective working relationships across the Health SystemAbility to maintain a work pace appropriate to the workloadMust demonstrate customer service skills appropriate to the jobAll external candidates must have a Bachelor of Science in NursingCurrent California RN licenseMinimum of 6 months demonstrated nursing experience in an acute care or ambulatory care settingAbility to read, write and communicate effectively in EnglishAbility to work variable shifts including evenings, nights, weekends, and holidaysAbility to maintain flexibility and work well in a constantly changing environmentMust possess the skill, knowledge and ability essential to the successful performance of assigned duties
