The UNC Infectious Diseases Clinic serves a diverse population living with or at risk for HIV and other infectious diseases. The Clinic specializes in treating people with infectious diseases such as HIV/AIDS, TB, other sexually transmitted diseases, and acute and chronic infections. The clinic also provides treatment of infections in returning travelers. Our consult service evaluates a variety of outpatient infectious diseases .Equal Opportunity Employer

This position provides comprehensive social work practitioner (SWP) services to a high volume of adults with HIV infection receiving medical care in the Infectious Diseases Clinic. A much smaller percentage of time involves provision of services to non-HIV patients with other infectious diseases. These services include comprehensive assessment of needs, medical case management, mental health and substance use screening and services, and linkage of clients with health care, psycho-social, and other resources. Medical case management includes the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments. Key activities include: (1) initial assessment of service needs, including triage of multiple patients and prioritization of tasks as needed; (2) development of a comprehensive, individualized care plan; (3) coordination of services required to implement the plan; (4) client monitoring to assess the efficacy of the plan; and (5) periodic reevaluation and adaptation of the plan, at least every 6 months, as necessary. It includes client-specific advocacy and review of utilization of services. This includes all types of case management, including face-to-face meetings, telephone calls, and any other forms of communication. Social Work Practitioner (SWP) will intervene to facilitate linkage, engagement and retention in medical care through individualized and systematic interventions. SWP will work collaboratively with an interdisciplinary team of medical providers, nursing staff, and financial counselors. SWP is expected to coordinate services with other community resources. SWP may conduct HIV testing with training. Position requires timely data entry and data management in electronic medical record and other databases and a strong ability to multi-task in a fast-paced environment. Tracking of service numbers, creation of periodic reports is expected. Position will assist in the development of policies, procedures and protocols to meet the needs of the clinic population. SWP will participate in clinical quality improvement activities. Position is expected to provide coverage for SWP colleagues as needed and attend monthly staff and quarterly provider meetings and other meetings as directed. The ability to provide clinical supervision for SWP to obtain licensure and certification is highly desirable.Minimum Education and Experience Requirements
Bachelor’s degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.Essential Skills, Knowledge and Abilities
Must possess excellent interpersonal and communication skills, both oral and written, which are required for interacting with large interdisciplinary team. Must act within a social work team environment, providing back-up for colleagues when needed. Must demonstrate flexibility to accommodate needs of patients and the social work team. Candidates must have the ability to analyze and interpret information needed to plan and prioritize appropriate interventions. Essential that applicants have sensitivity to and are respectful of people living with HIV/AIDS, as well as racial and sexual minorities. Must possess knowledge of social services procedures, policies and resources and have the ability to navigate patients and families through institutional systems (department of social services, health departments, medical home, etc.). Applicants should have the knowledge and experience to counsel patient and family. Ability to communicate effectively via e-mails and Epic staff messages.Preferred Qualifications
Strong preference will be given to those who have worked in the HIV field. Supervisory experience is preferred. Additional training or certification in mental health or substance abuse is desirable.
Prefer candidates with a Master of Social Work (MSW) degree. Licensed Clinical Social Worker (LCSW) preferred. Prefer candidate with the ability to supervise other Social Work Practitioners. Experience with electronic medical records and data management strongly preferred.
