Job Description:

The Department of Pediatrics seeks dedicated and caring diabetes peer coaches to work with medically underserved communities with Type 1 Diabetes and Type 2 Diabetes throughout Florida. This part-time diabetes peer coach position in Dade County, Florida is funded through the Extension for Community Health Outcomes Type 1 Diabetes and Type 2 Diabetes grant and are time-limited positions related to the goals of the pilot/feasibility stage of the project.

Diabetes peer mentors will work with adults who have Type 1 Diabetes (T1D) or Type 2 Diabetes (T2D) in clinical primary care settings and will:

(1) help facilitate better care navigation

(2) help connect patients with T1D or T2D to community support and outreach opportunities in their area

(3) help connect patients living with T1D or T2D to other patients with T1D or T2D in their local communities and

(4) help connect participating ECHO T1D and T2D sites (“spokes”) with UF (“hub”) through participation in weekly tele-education clinics.

The diabetes peer coaches will work in team settings with clinical and academic faculty to achieve the goals of the grant and will work under the supervision of the Principle Investigator for the ECHO grant, Dr. Michael Haller.

Advertised Salary:

$16-20 per hour, commensurate with experience

Minimum Requirements:
  • Associate's degree or higher
  • Strong communication skills
  • Excellent interpersonal kills
  • Ability to work in team settings
  • Ability to work with people from diverse backgrounds
Preferred Qualifications:
  • Strong working knowledge of Type 1 Diabetes and Type 2 Diabetes
  • Strong working knowledge of clinical settings
  • Local to the community being served
  • Have mentoring, teaching, and/or community health worker experience
  • Fluency in Spanish or Creole speaking a plus
