  • Position Description:

    The Clinic Nurse Case Manager provides a collaborativeprocess of assessment, planning, facilitation, care coordination, evaluationand advocacy for options and services to meet each patient’s needs to achieveoptimal clinical, financial and satisfaction patient outcomes.  The overall goal is to enhance the quality ofpatient care throughout the continuum of care to achieve excellence in healthcaredelivery, within a framework of fiscal responsibility.

    The function of this position is to triage patient phonecalls for the majority of the physicians in the department.  It is expected that this position relieve thephysician, nurse practitioners, and physician assistants of routine phone callsand other tasks that do not necessarily have to be completed by a physician,including prescription refills, document completion, and patientauthorizations.  This position isexpected to work independently and have excellent customer service andcompassion for patients.

    Qualifications:

    Qualifications/Knowledge, Skills & Abilities:

    • Current RN licensure recognized by the Stateof Ohio.
    • BSN required.
    • Minimum of 3 years clinical experiencerequired – preferably in specialty field.
    • Possess organizational, problem-solving,and critical thinking skills.
    • Demonstrates ability to work withself-direction and effectiveness under minimal supervision.
    • Demonstratesexcellent written, verbal & group communication skills.
    • CPR Certification required.

    Communication and other skills:   

    • Able todocument care legibly in hand written text.
    • Able tonavigate through the EMR and record required data accurately and efficiently.
    • Able tocommunicate and collaborate with team members including but not limited to physicians,nurses, managers, MA's and other team members.

    Working Conditions:1. Able to work flexible hours, shifts, holidays and weekends. 2. Tolerate busy office environment that requires constant interaction with patients, employees, students, residents, and physicians. 3. Prolonged walking and standing. Occasional lifting of patients sometimes over 200 pounds.4. Occasional carrying of items weighing up to 30 pounds required. 5. Requires corrected vision and hearing to normal range and manual dexterity.6. Ability to operate various types of office equipment, including but not limited to: Personal computer, fax machines, copiers, telephones7. Exposure to communicable diseases and/or body fluids, toxic substances, medicinal preparations and other conditions common to a clinic environment on a daily basis.8. Able to work effectively both with internal and external customers in stressful situations.
