Family Support WorkerJob Description:
The Family Support Worker works as a Peer Navigator in a team setting to provide a bridge between providers and HIV Infected patients to facilitate the medical and psychosocial care of the patient. Works to increase consumer participation in program planning and implementation including Community Advisory Board (CAB) and support groups.
- May provide outreach work.
- Individual contact with patients to identify and develop client directed treatment plan goals and monitor ongoing achievement of goals.
- Provide peer support.
- Perform other duties to enhance consumer confidence, involvement and empowerment, including attendance at budgeted training sessions.
- Provide administrative assistance with clinical operations and medical records as needed, including scanning electronic medical records.
$14.00 - $18.00 an hour.Minimum Requirements:
High school diploma or equivalent and one year of experience working with children and families.Preferred Qualifications:
At least 1-2 years' experience with HIV population preferred, either via work environment or participation in volunteer activities. Basic computer proficiency.Special Instructions to Applicants:
