The Division of Infectious Diseases at the University of Virginia is seeking a Case Management Supervisor (CMS) to join the Ryan White Clinic team. The CMS is a key member of the leadership team at the federally-funded UVA Ryan White Clinic. This person oversees the team of individuals who provide comprehensive psychosocial support services to our patients. This person is responsible for supervising the Ryan White Program Case Management team including, Medical Case managers, Non-Medical Case Managers, Community Health workers, and Housing Coordinator. The candidate will supervise multiple staff members and will directly be involved in decision-making for hiring, Performance Management, and discipline. He/She will ensure that the Case Management office is staffed appropriately and that daily operations operate according to the UVA Health System and Ryan White Program procedures. The Case Management Supervisor will also coordinate the patient caseload assignments, the day-to-day delivery of patient care coordination, and case management services. In addition, he/she will make sure that the staff are working with patients to develop income-based case plans that address their barriers to securing and maintaining safe stable housing.

  • Oversee the clinical and administrative operations of the Medical Case Management, Non-Medical Case Management, and Community Health workers Services.
  • Provide regular supervision to case management staff with diverse areas of expertise including job development.
  • Ensure that Case Managers continually monitor and evaluate each patient’s progression through their case management plan, developing corrective action revisions to the plan as needed.
  • Ensure the timely flow of Case Management activities to meet program goals and objectives.
  • Oversee the integration of trauma-informed and gender-responsive services for men and women participants.
  • Use motivational interviewing and supervise staff in use of motivational interviewing to engage and retain patients in services.
  • Allocate time to direct patient care services as a member of the clinical team, including direct work in the community and with staff as needed.
  • Assess patient immediate needs, legal circumstances, and engagement needs to develop service plans.
  • Complete assessments, progress notes and significant program data.
  • Delegate and assign responsibilities to Case Management office staff and assess employee performance as needed.
  • Develop and implement office policies and procedures, helping to foster a culture of compliance.
  • Formulate objectives and processes for the Case Management Team to improve efficiency and ensure that daily operations run smoothly.
  • Complete required grant reporting timely and collaborate with others on the team on developing grant reports.
  • In addition to the above job responsibilities, other duties may be assigned.

MINIMUM REQUIREMENTS

  • Master’s Degree in Social Work, Psychology, Counseling, Education or Sociology and at least four years of increasingly responsible employment in related field is preferred.
  • Management experience in social service setting with working knowledge of Case Management systems and planning techniques. At least ten years of administrative experience in a health care environment is preferred.
  • Knowledge of substance abuse recovery and mental health care necessary.
  • Experience working with homeless individuals and families a plus.
  • Experience in administering public funding is desirable.
