Job Summary:
As a member of the multidisciplinary health care team, the ClinicalCase Manager assesses, plans, implements, coordinates, monitors, and evaluatesoptions and services to meet an individual’s health care needs throughcommunication and available resources to promote quality cost-effectiveoutcomes. The Clinical Case Manager facilitatesaccess to all core medical services and coordinates comprehensive HIV primarycare service delivery planning for patients through assessment, servicefacilitation, referral, advocacy, and follow-up. By working collaborativelywith the patient and the health care team, and utilizing established Levels ofService Criteria, the Clinical Case Manager plans and implements with thepatient a Comprehensive Medical and Health-related Services Care CoordinationPlan, updates the plan as needed, documents all care and service contacts,monitors patient progress, and continually reassesses the individual's need forlevel of service. The Clinical Case Manager provides treatment adherencereadiness counseling and treatment education and support, in collaboration withthe other members of the health care team.

Physical Requirements:
Ability to lift, carry, push and pull 10 pounds, reach, finger, grasp, feel, talk, hear, see and perform repetitive motions with or without reasonable accommodations.

Education & Experience

Position Qualifications:

Required:
-Masterslevel degree in Counseling Psych (MS), Masters level degree in Social Work (MSW) or an undergraduate degree inNursing (BSN required). Other equivalent educational qualifications/experience combinationswill be considered.
-ForMS/MSW, licensure is preferred (LCSW or LPC). (For RN, possession of validtemporary or permanent professional nursing license issued by the OklahomaState Board of Nurse Registration and Education is required).
-Must be able to travel locally within the 918 areaof Oklahoma,have dependable transportation, and be sensitive to rural communities andcommunities of color, gay/lesbian/bisexual communities, and persons who havebeen or are substance using/abusing, or are incarcerated or homeless.

Skills, Proficiencies, Knowledge:
Knowledgeof and experience with local health and social services care coordination,including eligibility assistance necessary. Excellent interpersonalcommunication skills, and written planning and documentation skills required.

Preferred:
-Twoto five years professional experience in health and human services setting(s),with previous case management experience preferred.
-Experiencein and knowledge of HIV direct care highly desirable.

Oklahoma State University-Center for Health Sciences was founded in 1972 and is home to the College of Osteopathic Medicine, as well as graduate programs in forensics, biomedicine, healthcare and athletic training.   Our core business is education; and it takes more than 500 faculty and staff members to run our institution.  Our mission is to educate and train osteopathic physicians, research scientists and other healthcare professionals with an emphasis on serving rural and underserved Oklahomans.  We offer more than 10 different specialty clinics to care for patients in the area.
