The clinic is housed within the Department of Medicine, Division of Infectious Diseases. People at risk for HIV/AIDS may be candidates for biomedical interventions that can significantly reduce the risk of HIV acquisition; Pre-exposure prophylaxis (PrEP) and/or Post-exposure prophylaxis (PEP). The PHC is seeking a Clinical Coordinator to contribute to the care navigation and development of these two very important service lines.

Duties & Responsibilities

The PrEP/PEP ClinicalCoordinator will support activities and functions of established PrEP/PEPprogramming to ensure that goals and objectives are accomplished in accordancewith existing priorities, time limitations (crucial), funding limitations andother specifications. The PrEP/PEP Clinical Coordinator will play a criticalrole in the provision of core medical and support services for patients whohave been referred to the PHC by self-referral or communitycollaborators.  The PrEP/PEP Clinical Coordinator will be responsible foridentifying the best course of action and facilitating linkage and access tocare as appropriate for those determined to be at ongoing or acute risk forHIV. The candidate must possess the ability to work with a wide variety ofpeople, including clinicians, pharmacists, patients and community partners, anddeliver prevention services, education and messaging in a non-judgmental andcaring manner. Core activities include: development of strategic partnerships,assessing patients for medical, psychosocial, and financial service needs,facilitating patient navigation of healthcare systems and promoting patientaccess to community resources.

The PrEP/PEP ClinicalCoordinator will act as the entry point for patients seeking PrEP and/or PEPand will be an essential part of the patient’s care team; which consists of thePrEP/PEP Clinical Coordinator, a pharmacist and a clinician.  The PrEP/PEPClinical Coordinator will act as the primary point of contact for coordinationof PrEP/PEP services at the PHC and the candidate should be comfortablediscussing HIV risk factors and providing health education related toprevention methodologies for HIV and sexually transmitted diseases other thanPrEP/PEP. The candidate should demonstrate experience and skill around thedevelopment of highly-coordinated treatment plans and be able to effectivelydiscuss such plans with both patients and licensed professionals. The idealcandidate will be well versed in the adult PrEP/PEP care guidelines,treatments, common socioeconomic barriers found in the patient population andbe familiar with the various state and federal programs that support access tocomprehensive healthcare. They should feel comfortable seeking collaborationsthroughout the community to aid in increasing access to HIV prevention.

Duties Include:

  • Serve as primary point of contact for PrEP/PEP participants and program partners.
  • Monitor incoming referrals (from collaborators, internal, or patient self-referral) and respond to care coordination needs in a timely fashion
  • Coordinate care for PrEP/PEP patients and facilitates follow-up care and referrals as directed by providers
  • Assess patient need for financial support in order to access medical care and medications and completes necessary applications
  • Monitor patient status changes through communication with clinical and support staff, community partners, and outside laboratories
  • Maintains security and confidentiality of sensitive information and documents actions in program database and EMR
  • Provide support for the PHC Treatment Adherence Program (medication acquisition, prior authorizations for patient assistance programs, and coordination of benefits, etc.)
  • Work closely with leadership staff to further develop and improve PrEP/PEP programming by conducting targeted outreach as necessary and participating in quarterly PrEP/PEP community collaboration meetings
  • Maintain program PrEP/PEP databases for accurate capture of referrals and outcome dispositions and provide quarterly and annual PrEP/PEP activity reports
Minimum Qualifications
  • Bachelor's degree or equivalent advanced learning attained through professional level experience required
  • Minimum of 5 years of relevant work experience is required
Preferred Qualifications-Bilingual English/Spanish. -Familiarity with biomedical HIV prevention interventions including updated clinical guidelines. -Familiarity with HIV prevention and care services. -Experience working in a clinical/hospital setting or with populations who are at high risk for HIV/AIDS or other disease states. -Knowledge of HIV/AIDS health education and local resources. -Experience with program development and/or quality improvement projects.
