Position Summary:

Outpatient Dialysis Nurse Practitioner will provide patient education, create dedicated care plans, and provide clinical guidance to an integrated care team all with the best interest of the patient as the guiding principle. Incumbent will work closely with the supervising nephrologist to provide superior care for our dialysis patient population. Incumbent will function autonomously, and in collaboration with all members of the healthcare team to coordinate and facilitate high quality, cost-effective care and will provide coordinated care services between the patient, family, physicians, additional providers, and other care team members. Responsibilities will also include providing dialysis rounds and documenting in the electronic medical record system.

Functions as appropriate to their “scope of practice” (In general, the “scope of practice” includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per their approved standardized procedure). Familiarity with state statute for their licenses is necessary.

Approved privileges are unique to each person and are unlikely to include ALL privileges

The NP professional may provide patient care in age groups based on educational preparation and at all Health licensed settings and affiliates.

The NP has responsibility for the standards of care for their specified patient population and evidences self-direction and accountability both to quality of care and in the development of their role within the all Health locations and affiliates.

Advanced Practice Practitioners (APP) are required to be credentialed to practice and be privileged through Medical Staff to function in their clinical role. Credentialing and approval of privileges must be completed prior to the first working day.

The NP is accountable to, evaluated, and provided feedback by the supervising/collaborating physician, physician designee(s) in the same clinical specialty, or the Administrative Manager.

Compensation Range:

$63.84 to $73.32

Master’s or Doctoral Degree in Nursing

Minimum of two (2) years’ experience as a Nurse Practitioner, preferred experience in Nephrology

Ability to obtain UCIMC Allied Health Professional Privileges prior to hire.

Demonstrates an understanding of and adheres to privacy, confidentiality, and security policies and procedures related to Protected Health Information (PHI) or other sensitive and personal information.

Skill to clearly communicate medical information to a broad variety of people, both in person and over the telephone.

Clear and professional communication skills; written and verbal

Ability to exercise diplomacy and flexibility in interacting with physicians, nurses, other paramedical personnel, and administrators

Certification by the State of California, Board of Registered Nursing as a Nurse Practitioner

Possession of an active and clear California License as a Registered Nurse

Acceptable Certifications:
NP National Board Certification (Beginning February 2018): Family/Individual FNP-BC or FNP-C
NP National Board Certification: Adult/Gerontology AGPCNP-BC or AGNP-C
NP National Board Certification: Adult Gerontology Acute Care AGACNP-BC or ACNPC-AG
NP National Board Certification: Adult NP-BC or Adult NP-C
Current, active, unrestricted Drug Enforcement Administration (DEA) license required
Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
