Cinical Patient NavigatorTUMG Medical Director

Location: New Orleans, LA

Summary

The Clinical Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes. The Navigator coordinates research data, community physicians, and Tulane University Health System resources into a seamless model of access and care benefiting patients, participating physicians, and family members. General duties of the position include, but are not limited to acting as patient advocate and assisting patients with navigating through healthcare system, facilitating health and disease patient education, conducts comprehensive, preventive screenings for patients and/or assists all support staff in daily patient interactions as needed, and facilitates patient medication management based upon standing orders and protocols.

Required Qualifications

  • RN with current license to practice in the State of Louisiana AND 3 years of clinical nursing experience to include experience in at least one of the following areas: managed care, patient education, patient advocacy/navigator, case management, or social work

Preferred Qualifications

  • Understanding of HMO and third party administration
  • More than 3 years of clinical patient navigator experience.
  • Understanding of home health, long term acute care, and rehabilitation care regulations
