The department of Pediatrics is seeking a Social Worker. The incumbent provides psychosocial assessments, intervention, and/or treatment and discharge planning to patients and families to assist them in coping with their hospitalization, illness, diagnosis, and/or life situation and meet the continuing needs of patients post hospitalization.

  • • Provides psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
  • • Formulates, develops, and implements a plan utilizing appropriate social work interventions.
  • • Collaborates with the health care team and involve the patient/family in the development and implementation of plans.
  • • Facilitates discharge planning across a multidisciplinary care continuum which includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan.
  • • Provides education and counseling to patients and families around issues related to adaptation to the patient's illness and/or life situation.
  • • Serves as a patient advocate.
  • • May utilize crisis intervention, brief and long-term individual, group, and family therapies.
  • • Assists with screening, identification and management of victims of abuse, neglect, domestic violence, rape, etc...
  • • Makes referrals as appropriate and serves as resource for the health care team.
  • • Maintains records and statistics in accordance with department policies.
  • • Attends and participates in staff, committee, department, and other administrative meetings.
  • • Provides education to other health care professionals.

Qualifications:
Bachelor’s degree in Social Work. Minimum 1 year of relevant experience Knowledge, Skills and Attitudes:
  • Skill in completing assignments accurately and with attention to detail
  • Ability to analyze, organize and prioritize work under pressure while meeting deadlines
  • Ability to process and handle confidential information with discretion
  • Ability to work evenings, nights, and weekends as necessary
  • Commitment to the University’s core values
  • Ability to work independently and/or in a collaborative environment
  • Proficiency in computer software (i.e. Microsoft Office)
